- $350,000 W-2 base salary
- 35% of collections; W-2 or 1099 options
- $1M+ earning potential based on historical practice performance
- $24,000 sign-on bonus
- Up to $25,000 relocation reimbursement
- $5,000 annual CE allowance
- Full benefits, 401(k) with 4% employer match, and employer-paid malpractice insurance
- Four weeks of PTO

- Full-scope oral and maxillofacial surgery
- Monday-Friday schedule
- No evenings, weekends, or on-call coverage
- Established referral network with significant growth potential
- Corporate marketing and operational support
- DDS/DMD from an accredited dental school
- Completion of an accredited OMS residency
- ABOMS board-certified or board-eligible
- Eligible for Kentucky dental licensure and the required sedation/general anesthesia permit
- Recent and upcoming residency graduates are welcome to apply. Out-of-state candidates and those needing additional relocation time will be considered.
