About the job:

The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.

What we are looking for:

• Education: Bachelor or Doctor of Pharmacy required

• Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio. Preferred - PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting

• Effective Communication Skills

• Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team

• Performs work independently with minimum supervision, and is effective in a stressful environment

• Individual driven to make a direct impact on patient care through our growing pharmacist-led Meds-to-Beds program, providing bedside counseling, discharge medication coordination, and transitions-of-care services.

• Innovative individual eager to make a meaningful impact through patient-facing clinical activities, including immunization services, medication counseling, and medication therapy optimization.

What you will do:

• Provide comprehensive pharmacy services, including the preparation, clinical verification, dispensing, and distribution of medications in compliance with state and federal regulations, accreditation standards, and organizational policies. Serve as a clinical resource and provide leadership to pharmacy support staff.

  Promote a culture of regulatory excellence by maintaining continuous survey readiness, ensuring compliance with all pharmacy regulations, and safeguarding accountability for medications, including controlled substances.

  Support pharmacy operations through effective inventory management, including formulary optimization, non-formulary medication review, expired product management, and oversight of medication utilization to ensure safe and efficient patient care.

  Perform clinical review, interpretation, and verification of medication orders to ensure safe, evidence-based, and effective medication therapy.

  Lead and support a pharmacist-driven Meds-to-Beds program, working directly with patients prior to discharge to improve medication access, provide individualized counseling, enhance medication adherence, and ensure a seamless transition from hospital to home. Pharmacists play a visible and impactful role in improving patient outcomes and reducing readmissions.

  Administer immunizations and provide vaccine education to eligible patients, helping improve community health outcomes while expanding pharmacist-led preventive care services.

  Collaborate closely with physicians, nurses, case managers, and other healthcare professionals to optimize patient outcomes through medication therapy management, antimicrobial stewardship, pharmacokinetic dosing, adverse drug event prevention, and evidence-based therapeutic recommendations.
