Role: Licensed Physical Therapist providing personalized in-home and senior living care to older adults, utilizing a flexible schedule and virtual care options.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and support aging in place.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency in EMR and basic computer skills, current CPR certification, and physical ability to perform in-home care duties including lifting, repositioning, and driving.
