Physical Therapist

Seeking licensed Physical Therapists to provide one-on-one care to older adults in private homes and senior living communities, using the innovative Geriatric House Calls™ model.

Key Responsibilities Include

• Performing evaluations, developing individualized treatment plans, and delivering evidence-based therapy sessions to enhance mobility, independence, and well-being.
• Documenting patient progress accurately within EMR systems and collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educating patients, caregivers, and staff to promote safety and aging in place.
• Incorporating telehealth and virtual care to increase access and flexibility.
  
  

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in basic computer skills, EMR use, and current CPR certification.
  
  

Physical requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and perform CPR, as well as drive between patient locations and wear PPE as needed.
