Position Overview: A licensed Physical Therapist responsible for delivering personalized in-home and senior living care to older adults, utilizing evidence-based treatments to improve mobility, independence, and quality of life.
Key Responsibilities
Provide one-on-one therapy in private residences and senior communities
Conduct evaluations, develop care plans, and deliver evidence-based treatments
Support patient wellness, mobility, and functional independence
Document care accurately in EMR systems
Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and staff on safety and aging in place
Incorporate telehealth and virtual care as appropriate
Qualifications & Skills
Valid Physical Therapy license or eligibility
Degree from an accredited program
Proficiency in computer use and EMR systems
Current CPR certification
Physical Requirements: Ability to perform moderate to heavy physical activities, including lifting, repositioning patients, walking, climbing stairs, and driving, with adherence to PPE protocols.
