Physical Therapist

Role Overview: This position is for a licensed Physical Therapist providing personalized, one-on-one care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model.

Key Responsibilities

• Deliver evidence-based physical therapy to support wellness, mobility, and independence
• Perform evaluations, develop individualized treatment plans, and document in EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Incorporate telehealth and virtual care to enhance access and flexibility
  
  

Qualifications & Skills

• Valid Physical Therapy license and degree from an accredited program
• Proficiency in computers and EMR systems
• Current CPR certification
• Ability to perform physical demands such as lifting, repositioning, and assisting with patient mobility
