We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in private homes and senior living communities.
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to enhance mobility, independence, and overall wellness.
- Document patient progress accurately within electronic medical records and collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth and virtual care to increase flexibility and patient access.
- Valid state license, degree from an accredited program, proficiency with EMR systems, and current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and driving between locations.
