Role: Licensed Physical Therapist providing personalized outpatient care to older adults in private homes and senior living communities.
Responsibilities: Conduct evaluations, develop care plans, deliver evidence-based therapy to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth as appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency in computer and EMR systems, current CPR certification, and ability to meet physical demands such as lifting and assisting patients.
Key Duties: Offer one-on-one therapy, support patient wellness, promote aging in place, and support community-based care.
