Physical Therapist
Seeking licensed Physical Therapists to provide one-on-one geriatric care in patients' homes and senior living communities, utilizing the signature Geriatric House Calls™ model. Responsibilities include performing evaluations, developing individualized treatment plans, delivering evidence-based therapy to enhance mobility and independence, and documenting in the EMR system. Collaboration with interdisciplinary teams and patient/caregiver education are key components. Telehealth and virtual care options are incorporated for greater flexibility and continuity of care.
Qualifications include a valid PT license, degree from an accredited program, computer proficiency, and CPR certification. Physical demands involve lifting, assisting with transfers, repositioning patients, and driving between care sites.
