Physical Therapist (Full-time)
Seeking a dedicated Physical Therapist to provide outpatient rehabilitation services in home and community settings for older adults. The role involves
assessing patient needs, developing and implementing individualized therapy programs, and making referrals to other disciplines as needed. Responsibilities include
performing comprehensive evaluations, applying therapeutic techniques such as therapeutic exercises, manual therapy, and modalities, and instructing patients and caregivers to promote

Qualifications include a valid PT license in the state, a Master’s degree in Physical Therapy or equivalent, and
