Role: Licensed Physical Therapist providing personalized in-home and senior living care to older adults, utilizing a flexible schedule and incorporating telehealth when appropriate.

Responsibilities

Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and overall well-being.
Document patient progress accurately within an EMR system, adhering to regulatory standards.
Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and staff to promote safety and aging in place.
Support virtual care initiatives to enhance access and continuity of care.

Qualifications & Skills

Valid Physical Therapy license or eligibility in the practice state.
Degree from an accredited Physical Therapy program.
Proficiency in basic computer skills and EMR systems.
Current CPR certification.
Ability to meet physical demands including lifting, repositioning patients, and driving.
