Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document care accurately in EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency in computer and EMR systems, and current CPR certification. Ability to meet physical demands such as lifting, assisting with transfers, and performing therapeutic activities. Must be able to drive to patient locations and wear PPE when needed.
