Physical Therapist (Sign-On Bonus)

Role Overview

A licensed Physical Therapist provides personalized, one-on-one care to older adults in their homes or senior living communities, supporting mobility, independence, and overall well-being. The position emphasizes autonomy, allowing therapists to create and manage their own treatment schedules and incorporate telehealth for flexible, continuous care.

Key Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions.
• Support patient wellness, functional independence, and longevity.
• Document clinical progress accurately within the EMR system.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual care to enhance access and flexibility.
  
  

Qualifications & Skills:

• Valid Physical Therapy license or eligibility.
• Degree from an accredited program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.
  
  

Physical & Other Requirements:

• Ability to perform moderate physical effort, including lifting and assisting with patient transfers.
• Driving to patient locations and performing therapeutic activities.
• Wearing PPE as needed.
