You will serve as the clinical leader of an interdisciplinary team, collaborating with nurses, social workers, and therapists to create comprehensive care plans that keep patients healthy, independent, and in their homes. Manageable Caseload: See only 8–10 patients per day, allowing for deep clinical assessment and relationship building. True Work-Life Balance: Maintain a standard 8:00 AM – 5:00 PM, Monday through Friday schedule. Enjoy your weekends and evenings with a phone-only, limited on-call rotation. No Hospital Rounding: Focus your expertise entirely on community-based care. Collaborative Leadership: Lead Interdisciplinary Team (IDT) meetings to craft and execute personalized, holistic care plans. Care Coordination: Oversee transitions of care, including acute episodes and long-term placement management. Advocacy: Act as the primary advocate for your patients within the Medicare and Medi-Cal systems.
