Working within a multidisciplinary team, the CNC plays a key role in delivering high-quality, patient-centred care, supporting nursing staff, and providing expert assessment, education, and care coordination for patients with cardiovascular disease, heart failure, arrhythmias, and cardiothoracic conditions across inpatient and outpatient settings.
