We are looking for Social Care Support Officers to support us to deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home. Working alongside partners, community organisations, carers, and most importantly the people we support to shape meaningful and personalised outcomes. Social Care Support Officers work across a range of teams and services in Adults Health and Wellbeing, including neighbourhoods, hospital and specialist settings. You will work with people with a wide range of experiences, strengths and support needs, in partnership with them, their families, carers and others important in their lives.
