The ACO Partner Performance Manager serves as the primary performance and relationship resource for assigned partner practices and partner facility operators participating in ACO REACH/LEAD and/or the Medicare Shared Savings Program (MSSP). This position translates financial, utilization, clinical, quality, attribution, and risk adjustment data into practical strategies that improve partner performance, strengthen care delivery, and support successful ACO outcomes.

The Manager builds trusted relationships with practice leaders, physicians, advanced practice providers, facility administrators, and operator leadership; coaches partners through performance improvement; and establishes clear goals, accountability, and follow-through. Working across Clinical Operations, Quality, Coding, Analytics, Revenue Cycle Management, and Contracting, the Manager aligns partner priorities with ACO objectives and coordinates the resources needed to execute measurable improvement plans.

The successful candidate brings at least three years of direct ACO REACH or MSSP experience, with three to five years or more preferred, strong analytical and communication skills, and demonstrated expertise coaching partner practices and facility operators to improve ACO performance. The role requires the ability to explain complex data clearly, influence clinical and operational leaders, and turn performance findings into sustained changes in partner workflows and outcomes.

Key Responsibilities

Partner Relationships and Accountability
  • Serve as the primary day-to-day contact and performance advisor for assigned partner practices and facility operators, building relationships with clinical, administrative, and executive leaders.
  • Understand each partner’s organizational structure, patient population, care settings, operating model, priorities, and barriers; tailor engagement and improvement strategies accordingly.
  • Establish shared performance expectations, define partner and ACO responsibilities, and maintain a consistent cadence of communication, follow-up, and escalation.
  • Identify partner concerns early, facilitate resolution, and escalate significant performance risks or operational barriers with clear recommendations.
Performance Analytics and Action Planning
  • Analyze and synthesize claims, financial, utilization, beneficiary attribution and alignment, quality, clinical, coding, and risk adjustment data at the ACO, operator, facility, practice, and practitioner levels.
  • Evaluate total cost of care, per-beneficiary-per-month (PBPM) spending, hospital admissions and readmissions, emergency department use, post-acute utilization, and other relevant performance indicators.
  • Compare results with applicable benchmarks, targets, and prior periods; identify variation, emerging trends, and the partners or patient populations requiring focused intervention.
  • Validate findings with Analytics and clinical teams, accounting for claims lag, data completeness, population changes, and other factors affecting interpretation.
  • Translate findings into prioritized performance improvement plans with defined actions, accountable owners, milestones, and measurable outcomes.
  • Monitor month-over-month and year-to-date progress, assess the impact of interventions, and revise plans when results fall short of established goals.
Practice and Facility Operator Coaching
  • Coach practice and facility operator leaders on how clinical workflows, care coordination, utilization management, documentation, and operational execution affect ACO performance.
  • Use performance reviews and workflow assessments to identify root causes, distinguish data issues from execution gaps, and agree on practical improvement priorities.
  • Guide partners in implementing strategies to reduce avoidable acute care use, improve transitions of care, close quality gaps, and strengthen coordination across physicians, facility staff, and care management teams.
  • Adapt coaching to each partner’s readiness, staffing, resources, and care setting, including skilled nursing and long-term care environments as applicable.
  • Provide constructive feedback, navigate difficult performance conversations, and build commitment to change while maintaining productive partner relationships.
  • Follow through on agreed actions, evaluate whether changes are adopted and sustained, and share successful practices across the partner network.
Partner Engagement and Education
  • Develop and manage partner engagement plans, meeting schedules, agendas, presentations, minutes, action items, and follow-up activities.
  • Lead structured performance reviews that clearly communicate results, explain the implications for ACO success, and secure agreement on next steps.
  • Develop and deliver training through live meetings, virtual sessions, webinars, and e-learning platforms; create presentations, job aids, and practical workflow guidance.
  • Facilitate Practice and Partner Roundtables and Quarterly ACO Partner Engagement Reviews, promoting peer learning and collaboration among practices and operators.
  • Tailor communications for executive, clinical, and operational audiences and assess education effectiveness through partner adoption and performance outcomes.
Quality Coding and Clinical Collaboration
  • Partner with Clinical Operations and Quality teams to implement clinical initiatives, improve care coordination, support quality measure performance, and address gaps in care.
  • Collaborate with Coding and provider education teams to support accurate, clinically substantiated diagnosis documentation, appropriate HCC capture, and compliant risk adjustment practices.
  • Use practitioner and facility performance findings to target education and improvement opportunities and coordinate specialist support as needed.
  • Support medical record review and quality reporting activities by coordinating partner access, follow-up, and corrective actions with the responsible teams.
ACO Program and Operational Coordination
  • Apply working knowledge of ACO REACH/LEAD and MSSP to help partners understand program objectives, performance drivers, and their operational responsibilities.
  • Coordinate with program and compliance teams to communicate applicable CMS requirements and program changes and help partners execute required activities on time.
  • Support beneficiary attribution and alignment review, partner roster accuracy, beneficiary identification and flagging workflows, and other program processes as applicable.
  • Coordinate with Revenue Cycle Management (RCM) to address billing workflows and operational issues affecting partner performance.
  • Liaise with Physician Contracting and Strategic Partner Management on provider additions, practice and facility changes, operator expansion, and participation needs.
  • Monitor relevant practitioner-level Quality Payment Program (QPP) activities with the responsible internal teams and maintain recurring partner and leadership reporting.
Required Qualifications
  • At least three years of direct experience supporting ACO REACH and MSSP performance, partner engagement, or operations; three to five years or more of relevant experience is preferred.
  • Demonstrated expertise coaching partner practices and partner facility operators to measurable improvements in financial, utilization, quality, clinical, or operational performance.
  • Strong analytical skills, including experience interpreting Medicare claims, financial and utilization reports, quality dashboards, attribution and alignment data, and risk adjustment information.
  • Strong written and verbal communication, presentation, facilitation, and interpersonal skills, with the ability to explain complex findings and influence action across clinical and operational audiences.
  • Working knowledge of value-based care, total cost of care, ACO performance benchmarks, beneficiary attribution and alignment, HCC risk adjustment, and quality improvement.
  • Proficiency with Microsoft Excel, PowerPoint, and healthcare reporting or analytics tools; ability to organize data, identify trends, and prepare clear performance presentations.
  • Experience developing and executing improvement plans, coordinating cross-functional work, and managing multiple partner relationships and deadlines.
  • Ability to work independently, exercise sound judgment, handle sensitive information appropriately, and hold partners accountable while sustaining trust.
Preferred Qualifications
  • Bachelor’s degree in healthcare administration, business, nursing, public health, or a related field, or an equivalent combination of education and relevant experience.
  • Experience working with skilled nursing facilities, long-term care operators, multi-site practices, and beneficiaries with complex medical needs.
  • Experience leading organizational change, provider education, workflow improvement, and executive-level performance reviews.
  • Familiarity with business intelligence platforms, electronic health records, care management systems, and population health analytics.
Measures of Success
  • Partners understand their performance, agree on improvement priorities, and complete assigned actions within established timelines.
  • Improvement plans demonstrate measurable progress in relevant cost, utilization, quality, and operational indicators, with appropriate consideration of population and data changes.
  • Practices and facility operators adopt and sustain agreed workflows, participate in performance reviews, and use data to guide decisions.
  • Internal teams receive accurate, timely reporting and clear escalation of risks, barriers, and resource needs.
