Nurse Practitioner or Physician Assistant (APP) – Headache & Migraine Specialist

The Opportunity: As an early Advanced Practice Provider (NP or PA) at Haven, you'll have an outsized impact on both our clinical care model and patient outcomes. You'll work directly with our multidisciplinary team—including our Stanford-trained Chief Medical Officer—to deliver high-touch, integrative care to patients navigating chronic headache disorders. We're tackling a $112B market opportunity where only 8% of patients currently receive adequate care, and you’ll be on the front lines of changing that statistic.

Location & Work Schedule:

  • 100% Remote: You can be based anywhere in the U.S., as long as you are available to work Pacific Standard Time (PST) hours.

  • Licensure: Active NP or PA license in California or Washington is required (holding both is a strong plus).

What You’ll Do:

Your primary focus will be delivering compassionate, top-tier virtual care while collaborating on continuous improvements to our clinical model.

  • Deliver Exceptional Telehealth Care: Manage virtual visits for patients with migraine and complex headache disorders using ICHD-3 diagnostic criteria.

  • Build Comprehensive Care Plans: Create personalized, integrative treatment strategies combining evidence-based pharmacology with non-pharmacologic lifestyle interventions (sleep, nutrition, stress, and trigger management).

  • Provide Continuous Support: Review digital headache diaries and communicate with patients between visits to offer real-time guidance and care adjustments.

  • Shape Clinical Innovation: Partner directly with clinical leadership and our product/operations teams to refine care protocols, test new patient engagement strategies, and enhance the digital care delivery platform.

  • (Optional) In-Person Procedures: If located near a partner site and interested, opportunities exist to perform or coordinate procedures like Botox for chronic migraine, nerve blocks, and trigger point injections.

What We’re Looking For:

  • Licensure: Board-certified Nurse Practitioner (NP) or Physician Assistant (PA) with active licensure in California or Washington (required).

  • Clinical Expertise: 3+ years of dedicated clinical experience in headache medicine, neurology, or a specialized outpatient setting with heavy headache management focus.

  • Telemedicine Aptitude: Comfortable navigating digital care platforms, asynchronous patient communication, and AI-assisted clinical workflows.

  • Product & Patient Mindset: Strong clinical judgment, high empathy, excellent written and verbal communication skills, and comfort operating in an agile, early-stage startup environment.

  • Growth Orientation: Eager to learn, wear multiple hats as we scale, and help establish standard-setting clinical care protocols.

Target Compensation: $135,000 – $160,000 / year (Full-Time)

  • Hybrid / Expanded Scope Note: For candidates located in or near our primary California metro areas (e.g., San Francisco, Greater Los Angeles, San Diego, or Orange County) who are qualified and interested in blending telehealth with optional in-person procedural care (e.g., Botox, nerve blocks, trigger point injections), additional compensation flexibility and higher target ranges are available.
