Salary Range: $90,000.00 To $110,000.00 Annually
Position Summary
The Application Analyst - Value-Based Services (Healthy Planet) is responsible for the implementation, configuration, maintenance, support, and optimization of Epic Healthy Planet applications and related Value-Based Care workflows. This role serves as a key resource for operational and clinical stakeholders by supporting population health initiatives, accountable care organization (ACO) activities, quality reporting, risk adjustment programs, and data-driven decision-making.
The ideal candidate will possess a strong understanding of Epic Healthy Planet, population health management, and value-based care operations. This individual will collaborate with internal teams to improve workflows, support system enhancements, and ensure Healthy Planet applications effectively support organizational and customer objectives.
Position Expectations
- Serve as a primary application support contact for end-users and stakeholders utilizing Epic Healthy Planet and related Value-Based Care applications.
- Configure, maintain, and optimize Healthy Planet functionality, including Managed Contracts, Rosters, Attribution, Registries, Data Marts, Metrics, and Dashboards.
- Support ACO operations and population health initiatives through application configuration, troubleshooting, and workflow optimization.
- Analyze and resolve application, workflow, and system-related issues impacting Healthy Planet functionality and reporting.
- Participate in the design, build, testing, implementation, and ongoing support of Epic Healthy Planet applications and enhancements.
- Support system testing activities, including the creation and execution of test scripts, validation of system changes, and coordination with business stakeholders.
- Manage data imports, file loads, mapping activities, and validation processes to ensure data integrity and system performance.
- Configure and maintain healthcare quality measure sets, including CMS, HEDIS, PCMH, and other Value-Based Care programs.
- Support risk adjustment and population health initiatives through the configuration and maintenance of HCC and CDPS risk models.
- Collaborate with operational, clinical, and technical teams to identify process improvement opportunities and optimize workflows.
- Partner with internal stakeholders and external vendors to troubleshoot system issues and evaluate new functionality and enhancements.
- Support annual quality measure updates and regulatory requirements in collaboration with the Value-Based Services team.
- Develop and maintain system documentation, workflows, procedures, and support materials.
- Monitor system performance and proactively identify opportunities to improve efficiency, reporting, and end-user adoption.
- Perform additional duties and responsibilities as assigned.
Skills and Abilities
- Strong knowledge and hands-on experience with Epic Healthy Planet applications and population health workflows.
- Understanding of Value-Based Care, Accountable Care Organizations (ACOs), Population Health Management, and Quality Improvement initiatives.
- Experience supporting Managed Contracts, Rosters, Attribution, Registries, Data Marts, Metrics, and Dashboards.
- Strong understanding of healthcare quality measures, including CMS, HEDIS, and PCMH reporting requirements.
- Knowledge of risk adjustment methodologies, including HCC and CDPS models.
- Ability to analyze and troubleshoot complex application, reporting, and workflow issues and recommend effective solutions.
- Experience supporting application configuration, maintenance, testing, upgrades, and optimization initiatives.
- Strong analytical, critical-thinking, and problem-solving skills.
- Excellent written and verbal communication skills with the ability to explain technical information to non-technical audiences.
- Strong interpersonal and active-listening skills with the ability to gather business requirements and translate those requirements into technical solutions.
- Ability to collaborate effectively across cross-functional teams while managing multiple priorities.
- Strong attention to detail and commitment to accuracy, quality, and continuous improvement.

Required
- Minimum of 2-3 years of experience supporting Epic Healthy Planet applications.
- Current EpicCare Ambulatory Certification required.
- Experience supporting ACO operations, population health programs, and value-based care initiatives.
- Experience working with healthcare quality measures, including CMS, HEDIS, and PCMH.
- Experience supporting data management, data mapping, file imports, and system configuration activities.
- Knowledge of healthcare information systems and healthcare industry best practices.
Preferred
- Bachelor's degree in Information Technology, Healthcare Administration, Health Informatics, Business Administration, or a related field, or an equivalent combination of education and relevant experience.
- Experience working within a Federally Qualified Health Center (FQHC) environment.
- Experience supporting Epic Healthy Planet in a population health, community health, or value-based care setting.
- Experience collaborating with Clinical Operations, Quality, Population Health, Revenue Cycle, and IT teams.
- Experience participating in Epic upgrades, implementations, optimization projects, and system enhancement initiatives.
- Familiarity with healthcare analytics, quality reporting, and dashboard development.
