Minimum Qualifications:

• Possess a Master's degree from an accredited college or university with a major in Psychology, Social Work, OR Counseling.
• Hold licensure as a Limited Licensed Psychologist (LLP), Licensed Psychologist (LP), Licensed Master of Social Work (LMSW), or Licensed Professional Counselor (LPC) from the State of Michigan.
• Must qualify as a Qualified Mental Health Professional (QMHP) in accordance with the Michigan Medicaid Provider Manual. QMHP qualification requires meeting the position's educational requirements AND one of the following criteria:

  • Specialized training, including fieldwork and/or internships associated with an academic curriculum in which the student worked directly with persons receiving mental health services; OR

  • At least one year of experience treating or working with individuals with mental illness.

• Have two or more years of related experience and/or training with older adults or gerontology certification.
• Valid driver’s license and auto insurance
• Knowledge and understanding of co-occurring disorders (mental health and substance use)

Essential Job Function:

1. Provides individual in home assessments, individual counseling services based on a strength-based model and develops individualized strength-based treatment plans.
2. Engages consumer and family in the treatment process and goal-setting. Identifies risk factors including lethality and responds to barriers to successful treatment.
3. Works constructively with consumer to reach agreed upon outcome, and coordinates care with internal and external providers
4. Provides non-emergent in-home assessments and psychotherapy for Senior Reach referral
5. Maintains appropriate professional standards and provides appropriate follow- up for clients.
6. Maintains up-to-date knowledge of community agencies, resources and other services for older adults.
7. Provide proactive outreach including presentations on issues that are key to the aging population, their care-givers, and professional organizations and non- traditional organizations that serve that population.
8. Responsible for accurate and timely data collection, documentation and reporting in accordance with Senior Services standards of practice and any funding requirements.
9. Attends mandatory in-services, on-line training and any individualized training plan as required.
10. Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach.
11. Attends supervision at times and intervals agreed upon with supervisor.
12. Meet required number of consumer service hours as determined by Network Director
13. Be a team player and exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
14. Maintains effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public
15. Effectively responds to client needs and problems, initiates and maintains positive interactions, timely response to phone calls, pages, email and other requests.
16. Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Participates in related committees and/or task forces as needed
17. Flexible work hours may be required occasionally.
18. Demonstrates knowledge of principles of trauma informed care.
19. Willingness to support and assist individuals with trauma related issues
