About Spartan Wellness

Spartan Wellness is a growing Canadian virtual healthcare organization focused on helping patients navigate complex care journeys through coordinated, accessible and patient-centred healthcare.

As our team continues to grow, we're looking for a Bilingual Clinical Care Coordinator to join our remote Operations team.

This is an ideal opportunity for someone who understands the healthcare environment but is interested in moving into a more administrative, care-coordination-focused position.

You may currently be an RPN, nurse, medical administrator, care coordinator, case manager, care manager or healthcare professional who enjoys organizing the pieces behind patient care and making sure nothing gets missed. Fluency in both English and French is required for this position.

The Role

The Clinical Care Coordinator helps ensure that patients move smoothly through every stage of their care journey. You'll work closely with Nurse Practitioners, patients, administrative team members, insurers and external healthcare providers to make sure appointments, documentation, referrals, prescriptions, labs, insurance requirements and follow-ups happen when they're supposed to.

Think of yourself as the person keeping all the moving pieces of a patient's care plan organized and on track.

You will not be managing employees. Instead, you'll be managing patient casework, documentation and follow-up.This is primarily an administrative role, but having a healthcare or clinical background is important because you'll regularly be reviewing medical documentation, prescriptions, diagnoses, treatment plans and insurance-related information.

What You'll Be Doing

Patient & Care Coordination:

  • Serve as a key point of contact for patients throughout their care journey
  • Work closely with Nurse Practitioners to ensure care plans are followed and next steps are completed
  • Track patient follow-ups, appointments, referrals, labs, imaging, prescriptions and outstanding clinical documentation
  • Coordinate external consultations, treatment providers and Independent Medical Examinations (IMEs)
  • Identify missing information or potential delays and proactively follow up
  • Help ensure patients don't fall through gaps in communication or administration
  • Support patients with virtual appointments, telehealth technology and navigating their next steps

Medical Administration & Documentation:

  • Review and organize medical documentation, clinical records and patient information
  • Gather historical medical records and required supporting documentation
  • Update digital patient records accurately
  • Review documents and letters being prepared for insurance providers
  • Track outstanding documentation and follow up with the appropriate patient, provider or insurer
  • Support the clinical team by keeping NP task lists and follow-ups organized
  • Help reduce administrative backlog for Nurse Practitioners
  • Maintain detailed trackers and ensure assigned tasks are completed

Insurance & Case Coordination:

  • Communicate with insurers and other third-party stakeholders regarding patient documentation and care requirements
  • Support documentation submitted to organizations such as Veterans Affairs Canada and Medavie Blue Cross
  • Identify missing documentation or barriers that may delay insurance approval
  • Coordinate IME documentation, appointments and reports
  • Assist with insurance-related follow-up, denials, appeals and outstanding information
  • Ensure documentation is complete and submitted within required timelines

Team Support:

You'll be part of Spartan's broader administrative team. Team members understand and support one another's responsibilities while also having specific areas they own. Your niche will have a stronger clinical and medical-administration focus, particularly around complex patient cases, clinical documentation, insurance coordination and follow-up.You'll work closely with Nurse Practitioners, administrative staff and operations leadership while independently managing your assigned workload.

What We're Looking For

Bilingual English/French is required.

We're looking for someone who is exceptionally organized, comfortable working with medical information and able to keep track of many moving pieces at once.

A background as an RPN or other regulated healthcare professional would be highly valued, but we're also interested in candidates with strong experience in:

  • Healthcare care coordination
  • Medical case management
  • Care management
  • Clinical administration
  • Medical office administration
  • Insurance-related healthcare coordination
  • Nursing or another clinical healthcare environment

International medical or healthcare experience may also be considered for candidates who are established in Canada and planning to remain long term.

You'll Be Successful Here If You...

  • Are an excellent multitasker and rarely let details fall through the cracks
  • Are comfortable speaking with both patients and healthcare professionals
  • Understand medical terminology and can confidently read clinical documentation
  • Can understand diagnoses, prescriptions, treatment plans and medical records
  • Love trackers, task lists and following things through to completion
  • Are proactive rather than waiting for someone to remind you about the next step
  • Communicate compassionately and professionally with patients
  • Are comfortable communicating with insurance providers and external organizations
  • Can manage complex casework independently
  • Have strong written communication and documentation skills
  • Are comfortable working in a fully remote environment
  • Enjoy being part of a collaborative team while owning your own area of responsibility

Experience with EMRs, Microsoft 365, telehealth platforms, scheduling systems and clinical documentation tools would be an asset.

Compensation & Benefits

  • 47,000–55,000 annual salary, based on experience
  • Extended health and dental benefits
  • $500 annual expense account
  • Fully remote position
  • Monday–Friday, approximately 8:00 a.m.–4:00 p.m. EST
  • Some flexibility during the week when needed
  • Supportive and collaborative team environment
  • Opportunity to grow alongside a rapidly growing healthcare organization
  • Exposure to virtual healthcare, integrative medicine, insurance coordination and evolving digital-healthcare models

Why Join Spartan Wellness?

This is an opportunity to use your healthcare knowledge in a different way.

If you've worked clinically but are ready to step away from traditional bedside or clinic-based care, or you're already working in care coordination and enjoy managing complex patient journeys, this position offers the opportunity to combine healthcare knowledge, organization, patient communication and problem-solving in a fully remote environment.

Spartan Wellness is growing, and we're looking for someone who wants to grow with us.

If you're bilingual in English and French, understand the healthcare environment and are the type of person who makes sure every detail gets followed through, we'd love to hear from you.
