Bilingual Patient Navigator
Boulder, Colorado | Full-Time, 37–40 Hours/Week | Hybrid | Non-Exempt
Pay Range: $25–$27/hour
This is a hybrid position based in Boulder, Colorado and requires weekly in-person work at Boulder Valley Health Center. Candidates must be able to reliably commute to our Boulder clinic.

Boulder Valley Health Center (BVHC) is seeking a bilingual Patient Navigator to connect patients with appointments, financial assistance, community resources, referrals, transportation, and follow-up support while helping them understand their care and available options.

You’ll work closely with patients, clinical teams, and community partners to improve access to care and help patients move through complex healthcare needs with clarity and confidence.

What You’ll Do
As our Patient Navigator, you will help patients access and move through care with confidence. This role connects patients to appointments, resources, financial assistance, referrals, and follow-up support while helping address barriers that may make care harder to access.
  • Serve as a knowledgeable resource on BVHC services, community programs, and local and national support resources.
  • Guide patients through their care journey, including appointments, treatment plans, referrals, follow-up, and coordination of complex care needs.
  • Identify and address barriers to care, connecting patients with transportation, housing, financial assistance, support groups, Medicaid, abortion funding, and other community resources.
  • Help patients understand medical recommendations, abnormal test results, and available options so they can make informed decisions about their care.
  • Provide compassionate, trauma-informed and patient-centered support, using motivational interviewing and goal-setting techniques while maintaining appropriate professional boundaries.
  • Develop and maintain accurate community resource directories and referral information.
  • Build relationships with community organizations and participate in outreach activities, including resource fairs and community events.
  • Maintain accurate, timely, and confidential documentation in the electronic health record (EHR).
  • Stay current on relevant programs, regulations, compliance requirements, and available patient resources.
  • Collaborate with clinical, administrative, and community partners to improve workflows, share relevant patient and program insights, and support organizational goals.
  • Complete or pursue Patient Navigator and/or Community Health Worker certification within the first year of employment.
  • Perform other related duties as assigned.
What We’re Looking For
Required Qualifications
  • Availability: Ability to work any required shifts:
    • Monday to Friday: 8:00 am – 5:15 pm
  • 1–2 years of experience in patient navigation, patient advocacy, case management, care coordination, or a related role, preferably in healthcare.
  • Knowledge of or experience with healthcare assistance programs such as Medicaid, Medicare, Title X, WWC, grant-funded programs, or other financial assistance resources.
  • Strong commitment to reproductive freedom, health equity, and inclusive healthcare, including abortion care.
  • Bilingual fluency in English and Spanish.
  • Strong written, verbal, and interpersonal communication skills.
  • Strong critical thinking, organization, and problem-solving skills with a patient-centered approach.
  • Ability to work effectively in a fast-paced, collaborative healthcare environment.
  • Strong attention to detail and ability to handle confidential and sensitive information.
  • Comfort using technology, including EHR systems, data tracking, reporting, and Microsoft Office or similar tools.
  • Ability to use inclusive and respectful language related to gender identity, sexual orientation, reproductive health, and diverse patient experiences.
  • High school diploma or GED.
Preferred Qualifications
  • Associate degree in public health, social work, psychology, or a healthcare-related field.
  • Certification as a Patient Navigator or Community Health Worker.
  • Experience serving transgender, gender-diverse, low-income, uninsured, or historically marginalized communities.
  • Experience with motivational interviewing or trauma-informed communication.
  • Knowledge of community resources in Boulder County or surrounding areas.
  • Prior use of Athena or similar EHR platform.
  • Strong customer service background, demonstrating compassion and empathy.
Physical Requirements
  • Ability to hear adequately in person and over the phone.
  • Ability to move and lift up to 20 pounds.
  • Ability to perform tasks requiring standing, walking, sitting, stooping, kneeling, bending, and climbing stairs with reasonable accommodation.
