The CAP Clinical Supervisor plays a critical role in facilitating and overseeing the delivery of case management services for medically fragile children and physically disabled adults with complex care needs who are at risk of institutionalization. These beneficiaries reside in private homes where their health, safety, and well-being can be maintained within budget constraints set by the Division of Medical Assistance.The CAP Clinical Supervisor ensures quality case management by supporting Case Managers, individuals being served, and their families, as well as the broader community. This role requires coordination with staff to maintain compliance with best practices and accepted case management approaches. Additionally, the Clinical Supervisor provides direct case management, consultation, training, and development to staff while serving as a liaison with state agencies to ensure effective service delivery.
DUTIES AND RESPONSIBILITIES:
- Service Delivery and Oversight:
- Complete initial and annual assessments, ensuring alignment with case management assignments and beneficiary needs.
- Facilitate and monitor the delivery of CAP case management services, maintaining high standards of quality and compliance.
- Oversee and ensure services comply with federal and state regulations, as well as Medicaid and CAP program guidelines.
- Provide Local Approval Reviews (LAR) for all Initials, Clinical Needs Reviews (CNRs), CIS/COS, and POCRs, ensuring all required uploads meet DHB timelines and standards.
- Conduct audits of case management charts to ensure compliance with federal and state regulations and quality standards.
- Training and Staff Development:
- Train new staff, focusing on clinical documentation, direct clinical support, and principal activities of case management (assessment, care planning, referral, linkage, monitoring and follow-up).
- Assist in developing and coordinating regional and individual training programs to enhance service quality.
- Provide ongoing coaching and leadership support to staff, emphasizing their value and contributions to the team.
- Develop and lead training initiatives to improve staff understanding of clinical practices, documentation standards, and compliance requirements.
- Case Management and Support:
- Assist with managing caseloads during transitions, vacations, and other periods of change to ensure continuity of care.
- Act as a resource for staff, providing consultation and support for challenging cases and clinical concerns.
- Review and approve clinical notes for accuracy, content, and compliance, providing feedback and guidance, as necessary.
- Plan and lead team meetings, addressing operational and clinical issues and fostering team collaboration.
- Quality Improvement and Reporting:
- Contribute to the development and monitoring of internal Quality Improvement programs.
- Oversee and track critical reports, including CNRs, RAIs (Requests for Additional Information), incident reports, and eCAP monitoring.
- Ensure timely submission of required reports related to beneficiary and staff outcomes as mandated by DHB and organizational policies.
- Monitor and enforce compliance with service delivery timelines and guidelines as set by DHB.
- Collaboration and Advocacy:
- Act as a liaison between the CAP program and state agencies to promote program alignment and resolve issues.
- Engage with community organizations to promote the CAP program and support its growth.
- Support staff in providing the fullest capacity of services to clients, ensuring alignment with person-centered care principles.
- All other duties as assigned.
SUPERVISORY RESPONSIBILITIES:
This position supervises clinical staff.
MINIMUM QUALIFICATIONS:
- Strong understanding of Medicaid, CAP program guidelines, and state and federal regulations.
- Proficiency with clinical documentation systems such as eCAP Business system, NCTracks, Workday, and PACEnet systems.
- Excellent leadership, organizational, and communication skills.
- Proven ability to mentor, train, and support Case Managers in clinical and operational practices.
- Ability to manage multiple priorities and ensure compliance with deadlines.
Education and Experience:
Candidates must meet one of the following criteria:
Social Work Background:
- Bachelor’s degree in social work from an accredited school of social work.
- Minimum of five years of directly related experience, preferably CAP case management, in the health or medical field (homecare, long-term care, or personal care).
Human Services Background:
- Bachelor’s degree in a human services or equivalent field from an accredited college or university.
- Minimum of five years of community experience, preferably case management, in the health or medical field (homecare, long-term care, or personal care).
- Completion of a NC Medicaid-certified training program within 90 calendar days of employment.
Nursing Background:
- Current North Carolina Registered Nurse (RN) license with a two-year or four-year nursing degree.
- Minimum of five years of case management experience, preferably CAP case management, in homecare, long-term care, or personal care.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
- Must be able to lift a minimum of 10 lbs.
- Must be able to pull a minimum of 20 lbs.
- Must be able to squat, kneel, crawl, crouch, climb, and stoop.
- Required to regularly stand and walk.
