Help Families Navigate Care with Confidence

At Mae Health, we're on a mission to improve maternal health outcomes by helping mothers, babies, and families access compassionate, culturally responsive healthcare and community-based support.

Our Care Coordinators play an important role in that mission. They help members navigate healthcare services, address barriers to care, connect with available resources, and receive the support they need throughout their maternal health journey.

We're interested in connecting with experienced healthcare professionals who are passionate about maternal health equity, skilled at coordinating services, and committed to making a meaningful difference in the lives of the members we serve.

The Opportunity

As a Care Coordinator, you will support members across Mae's Medicaid health plan partnerships, helping them navigate healthcare needs and access appropriate clinical and community resources.

You will conduct comprehensive needs assessments, coordinate care plans and referrals, maintain accurate documentation, and serve as a liaison between members, Mae's internal teams, and partner care management teams.

Success in this role requires compassion, sound judgment, strong organizational skills, and the ability to manage multiple member needs while maintaining consistent outreach and follow-through.

What You'll Own

Member Support & Care Coordination

  • Conduct comprehensive member needs assessments addressing physical, medical, behavioral, psychosocial, environmental, and financial factors.
  • Identify barriers to care and connect members with appropriate healthcare services, community programs, and available resources.
  • Support members in participating actively in healthcare decisions with their providers.
  • Coordinate care plans, referrals, and follow-up activities with payer care management teams.
  • Identify and escalate high-risk factors, unmet needs, and quality-of-care concerns through established channels.

Member Engagement & Documentation

  • Build trusted relationships with members through compassionate, culturally responsive communication.
  • Use motivational communication techniques to encourage participation and follow-through.
  • Conduct outreach by telephone, text, and email to maintain member engagement.
  • Document assessments, outreach activities, referrals, and member interactions accurately in designated systems.
  • Maintain consistent communication with members and relevant care partners.

Collaboration & Accountability

  • Serve as a liaison between members, Mae's internal teams, and external healthcare partners.
  • Coordinate across multiple workstreams and member populations while managing priorities effectively.
  • Identify recurring barriers to care and recommend opportunities to improve the member experience.
  • Collaborate with colleagues to ensure a coordinated, member-centered approach to care.

What Success Looks Like

Care Coordinators are expected to balance high-quality member support with consistent outreach, accurate documentation, and timely care coordination.

Key expectations include:

  • Complete a minimum of 50 outreach attempts per day through a combination of telephone calls, text messages, and emails.
  • Maintain accurate and timely documentation of member interactions and care coordination activities.
  • Ensure referrals and follow-up activities are managed appropriately.
  • Build positive relationships with members and care management partners.
  • Escalate high-risk situations and care concerns through established procedures.
  • Demonstrate consistent progress toward member engagement and operational goals.

Experience We're Looking For

Required Qualifications

  • 3–5 years of experience in social work, nursing, healthcare, care coordination, or a related field.
  • Knowledge of care coordination, referral management, and community or public assistance resources.
  • Experience building relationships with patients, members, clients, or healthcare partners.
  • Strong organizational skills and the ability to manage competing priorities.
  • Excellent written and verbal communication skills.
  • Comfort conducting frequent telephone outreach and maintaining accurate electronic records.
  • Proficiency with digital systems used for communication, documentation, and care coordination.
  • Ability to work independently and collaboratively in a remote environment.

Preferred Qualifications

  • Experience supporting Medicaid populations or health plan members.
  • Background in maternal health, community health, or social services.
  • Experience working with doulas, birth workers, or community-based maternal health programs.
  • Additional language proficiency relevant to the populations Mae serves.
