Care Management Coordinator (RN)

Remote | California

Position Overview

We are seeking an experienced and highly motivated Care Management Coordinator to provide leadership and operational oversight for an Adult Case Management program. This role is responsible for supervising care management staff, driving performance and quality initiatives, and ensuring the delivery of effective, member-centered services.

The ideal candidate will be a licensed Registered Nurse with a strong background in case management, managed care, and team leadership. This individual will play a key role in supporting staff development, optimizing workflows, fostering collaboration across multidisciplinary teams, and enhancing health outcomes for the populations served.

Key Responsibilities

  • Provide leadership, supervision, and support to Adult Case Management staff in a primarily remote environment.
  • Monitor team performance, productivity, and operational metrics to ensure compliance with organizational standards and program goals.
  • Facilitate effective communication between care management staff, members, healthcare providers, and community partners.
  • Deliver coaching, mentoring, training, and professional development opportunities for clinical and non-clinical team members.
  • Address personnel and operational issues in a timely and professional manner.
  • Identify, develop, and implement process improvements to enhance program effectiveness and operational efficiency.
  • Support utilization review and case management activities to ensure appropriate, quality-driven, and cost-effective care delivery.
  • Collaborate with interdisciplinary teams and stakeholders to coordinate services and improve member outcomes.
  • Participate in quality assurance initiatives, audits, and compliance activities.
  • Foster a culture of accountability, collaboration, continuous improvement, and service excellence.

Required Qualifications

  • Active, unrestricted California Registered Nurse (RN) license.
  • Bachelor's degree required; Nursing, Public Health, Community Health, or a related healthcare field preferred.
  • Minimum of five (5) years of clinical healthcare experience.
  • Minimum of three (3) years of supervisory, leadership, or team management experience.
  • Minimum of two (2) years of managed care experience.
  • Demonstrated experience in medical case management and utilization review.
  • Strong understanding of population health, care coordination, and healthcare delivery systems.
  • Proficiency with electronic medical records, documentation platforms, and Microsoft Office applications.
  • Excellent communication, organizational, and interpersonal skills.

Preferred Qualifications

  • Experience working within Medi-Cal managed care programs.
  • background collaborating with community-based organizations and healthcare providers.
  • Proven success leading clinical teams and developing staff through coaching and mentorship.
  • Experience implementing quality improvement initiatives and operational enhancements.
  • Ability to effectively manage multiple priorities in a dynamic, remote work environment.

Work Environment

This position is primarily remote and requires frequent use of electronic health records, reporting systems, and virtual communication platforms. Quarterly onsite attendance may be required for team meetings, training activities, and organizational initiatives.

Job Type & Location

This is a Contract position based out of Santa Barbara, CA.

Pay and Benefits

The pay range for this position is $50.00 - $55.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
