About the role
We’re hiring a Care Navigator / Patient Advocate to work closely with patients and help them navigate their healthcare journey. This is a patient-facing role for someone who is deeply empathetic, highly organized, and comfortable using technology throughout the day.
This role is a strong fit for someone who loves helping people, communicates warmly and clearly, and can stay on top of tasks, follow-ups, and changing priorities in a fast-moving environment.
What you’ll do
Build trusting relationships with patients through consistent, supportive outreach
Help patients navigate appointments, referrals, care plans, and next steps
Provide high-touch support that makes patients feel seen, heard, and cared for
Coordinate across systems and workflows to keep patient needs moving forward
Document interactions carefully and maintain strong follow-through
Collaborate with a growing team to improve processes and patient experience
Adapt as tools, workflows, and internal systems evolve

Excellent communication and interpersonal skills
Genuine empathy and a strong instinct for patient-centered support
Strong organizational skills and attention to detail
Comfort working in multiple browser tabs, platforms, and digital tools
Resilience and adaptability in a startup-style environment
Ability to balance warmth with efficiency and execution
