Join the NVFS Family: We are looking to hire a Case Manager to join our Community Housing team's Rapid Rehousing (RHAP) program! In this role, you will help individuals and families experiencing homelessness or housing instability secure and maintain safe, stable housing. Through individualized case management, housing assistance, and community partnerships, you will support clients in overcoming barriers and working toward long-term self-sufficiency.

This position has a starting salary in the high 40s and low 50s. This position offers a hybrid work schedule, including remote work, office-based responsibilities, and visits to clients' homes and community locations.

In this role, you will be responsible for:

Support Clients and Families

  • Conduct client screenings, assessments, and intake appointments to determine eligibility for housing assistance and supportive services.

  • Provide individualized case management, develop service plans, and help clients achieve housing, financial, employment, and personal goals.

  • Connect clients with housing assistance, healthcare, employment, education, and other community resources.

  • Conduct home visits, monitor client progress, and complete reassessments to support ongoing housing stability.

  • Facilitate educational and skill-building activities focused on budgeting, tenant responsibilities, and independent living.

Build Housing and Community Partnerships

  • Collaborate with landlords, property managers, and community organizations to identify housing opportunities and support successful placements.

  • Assist with landlord-tenant communication, mediation, and housing-related concerns.

  • Conduct housing inspections and coordinate rental assistance, utility payments, and other eligible financial support.

Maintain High-Quality Services

  • Maintain accurate and timely case notes, client records, assessments, and required program documentation.

  • Ensure services comply with program guidelines, funding requirements, and housing standards.

  • Participate in team meetings, supervision, professional development, and collaborative program activities.

Your Background: We understand that you may obtain skills through a variety of professional, personal, educational, and volunteer experiences.

    • Prefer one of the following:
      • Bachelor’s degree in Social Work or related human services field; OR
      • Associate’s degree with at least two (2) years of related work experience OR
      • At least four (4) years of related work experience with no relevant education
    • At least one (1) year of case management experience specializing in human services; strongly prefer experience providing direct services to individuals in housing-related crisis
    • At least one (1) year of Homeless Management Information System (HMIS) experience preferred
    • Demonstrated ability to maintain accurate, timely, and relevant reports, case notes, forms, and documentation to support client services and resource referrals.
    • Must be able to communicate and interact effectively with a diverse client base and landlords
    • Housing Counselor and/or HQS (Housing Quality Standards) Certification a plus but not required
    • Oral and written fluency in English required; Spanish language fluency preferred. Additional language skills always welcome.

                        Additional Requirements:

                        • Successful completion of background checks upon hire.
                        • Must have reliable transportation to visit client homes and/or other designated community locations
                        • May be called upon to work beyond normal working hours
                        • Must be comfortable with varying home conditions.
