Position Overview & Organizational Relationships The Housing Stability Advocate/Case Manager is a specialized role within the "Pathways to Stability" innovation, designed to drastically decrease homelessness recidivism in the City of Albany. Utilizing an enhanced Critical Time Intervention (CTI) model, this position provides the "warm hand-off" necessary for households transitioning from crisis to independence. The Advocate bridges the "support cliff" experienced by households as they transition from emergency homeless locations, such as shelter or unhoused on the street, and move into permanent housing. The Advocate will provide continuity of care for families, youth, and criminal-justice-involved individuals, ensuring they master their "new normal" as they move into and maintain permanent housing through intensive, granular support and landlord mediation. This role is responsible for helping clients master their "new normal" by navigating granular tasks that often trigger instability, including managing changing Medicaid and SNAP regulations, maintaining eligibility for public or temporary assistance, gaining and maintaining employment, locating and accessing food pantry access, arranging school transportation for their children, and other necessary supports during a time of transition. Acting as a primary liaison, the Advocate serves as a safety valve for landlords through immediate mediation and the establishment of payment plans to prevent eviction. By providing intensive case management in areas like budgeting, home maintenance, and tenant rights, the Advocate empowers households to stabilize the conditions that led to homelessness and ensures long-term, cross-program sustainability. Requirements & Qualifications High School Diploma or equivalent (GED/ TASC) required; College degree highly preferred. A minimum of one-year previous experience working in homeless services, or human services. Flexibility schedule availability , including the ability to work outside regularly scheduled hours when necessary. Demonstrated ability to work collaboratively within a team and build strong rapport with clients. Clean and Valid New York State Driver’s License (actively licensed for at least 1 year) with reliable transportation*. Excellent organizational, planning, time management, and written communication skills . Strong computer skills , including proficiency in Microsoft Office Suite, Google Workspace, and secure internet practices. Warm, empathetic, and trauma-informed approach , with the ability to provide compassionate care to effectively support and empower clients in need. High ethical standards , exercising sound independent judgment, discretion, and strict client confidentiality. Reliable, self-motivated, and adaptable to fast-paced, day-to-day operational changes. *Staff in driving positions must be able to meet the agency’s minimum requirements of our insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire, and the ability to maintain insurability throughout employment. Essential Duties & Responsibilities Documentation & Compliance: Complete comprehensive intake and ongoing assessments, establish Service Plans with concrete, time-bound goals, and ensure timely, accurate documentation of service delivery (including progress notes and statistics) in the electronic health record. Life Skills Coaching & Housing Stability: Provide 1-on-1 teaching, coaching, and rehabilitative counseling to help households build the skills needed to maintain an apartment—including monthly budgeting, home care skills, and managing behavioral health or developmental barriers. Landlord Relations & Mediation: Serve as a household advocate by building and maintaining positive landlord relationships and providing conflict mediation while upholding professional boundaries. Service Coordination & Advocacy: Coordinate service plan delivery through ongoing communication with area providers, families, and community networks to connect households with mental health, education, employment, and non-healthcare resources. Client Engagement & Person-Centered Care: Engage directly with youth and households in their homes and community settings using a positive, strength-based, person-centered, and trauma-informed approach. Crisis Intervention: Intervene in crises using Therapeutic Crisis Intervention (TCI) strategies and assist with conflict resolution. Transportation & Field Support: Travel locally to provide in-home/community services and transport clients to appointments (e.g., Department of Social Services, employment resources, Social Security Administration, and food pantries). Professional Development & Growth: Attend all mandated agency trainings assigned by the supervisor. General Agency Support: Provide additional support and rehabilitative services based on new contracts, changing regulations, and other position-related duties assigned by the supervisor.
