Description
A Case Manager facilities a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes. This position will float between Spring Hill and Brooksville facilities.
Qualifications
Education: Associate’s degree in Nursing.
Experience: Two years’ clinical experience as a Registered Nurse. 1-2 years experience in Case Management preferred
Licenses/Certifications: Registered RN in the State of Florida, BLS required.
