Key Responsibilities

Come work with the Community Transition Waiver team to:

• Provide case management to people who transition from Intermediate Care Facilities into Home and Community-Based Services.
• Provide education to individuals and families regarding Home and Community-Based Services and available service options.
• Provide information concerning rights, options, benefits, services, Person Centered Planning goals and expectations.
• Complete field work assignments used to implement Person Centered Support Plans (PCSP), budgets, RFS tasks, Charting the Life Course, and UCANS assessments.
• Coordinate and refer services of community resources, placement providers, collateral agencies, the courts, and/or other community partners.
• Collaborate with other state and local programs, organizations and agencies, and with various stakeholders to promote program goals and objectives.
• Provide consultation, make recommendations, give appropriate advice, and/or assist to facilitate decisions.
  
  

Qualifications
Minimum or "day-one" qualifications:

• Has a Bachelor's Degree with at least 1 year of relevant experience working directly with people with disabilities.
• Has knowledge of Home and Community Based Services (HCBS) and Settings Rule.

• Has obtained certification as a QIDP (Qualified Intellectual Disability Professional).

Preferred or trainable/on-the-job qualifications:

• Has strong interpersonal skills.
• Is passionate about empowering people with disabilities with working knowledge of Home and Community-Based Services (HCBS), the Community Transition Waiver and HCBS Settings Rule
• Is an excellent listener who is also very proactive
  
  
• Has knowledge of community resources for people with disabilities and how to access those resources
  
  
• Has the ability to work well with a wide range of people with varying interests
• Can prioritize multiple tasks while maintaining attention to detail
• Is self-motivated with ability to work a flexible schedule
