About the Role

The Director, Clinical Performance serves as Chief of Staff to the Chief Health Officer and plays a critical role in translating clinical strategy into coordinated execution across Medical Practice, Behavioral Health, Pharmacy, Mobile Integrated Care, and Clinical Operations.

This role creates the operating systems, performance visibility, and cross-functional coordination needed to help the clinical organization execute against its strategic priorities. The Director will build and manage clinical performance reporting, lead cross-functional initiatives, support network strategy execution, provide leadership for the clinical learning strategy, and run the operating cadence for the Chief Health Officer and clinical leadership team.

The ideal candidate is an experienced clinical operations leader who can work across functions and markets, translate complex clinical and financial data into actionable insights, and drive execution through influence and strong cross-functional partnerships.

What You’ll Do

Clinical Performance & Reporting

  • Build and own a consolidated clinical performance scorecard covering quality, burden of illness, total cost of care, engagement, member experience, and team experience metrics.
  • Establish a consistent monthly and quarterly performance review cadence across the clinical organization.
  • Partner with Data and Analytics to apply enterprise KPI definitions and identify and resolve gaps or inconsistencies in metric definitions.
  • Lead quarterly analysis connecting clinical performance to financial performance and identify opportunities for performance improvement.
  • Surface trends, outliers, risks, and potential root causes to support the Chief Health Officer and clinical leaders in making informed decisions.
  • Develop and manage an intake process for clinical technology needs, coordinating requests across clinical teams, Integrated Care, and Technology and providing visibility into roadmap and delivery status.
  • Report team experience metrics for the clinical organization and support follow-through on initiatives identified through employee engagement and experience data.

Network Strategy & Clinical Performance Improvement

  • Lead execution of clinically owned components of the network management strategy in partnership with clinical, analytics, and value creation teams.
  • Partner with Data and Analytics to identify high-volume external referral patterns and establish baselines for measuring improvement.
  • Lead the clinical workflow for eConsult initiatives, including utilization monitoring, provider adoption, and performance.
  • Track and report market-level progress in reducing avoidable external specialty referrals.
  • Partner with Value Creation and Integrated Care on value-based specialty arrangements, providing clinical requirements, referral volume insights, and care model considerations to inform strategy and vendor selection.
  • Support development of provider-facing approaches that promote appropriate, conservative-first care pathways while maintaining member trust and quality of care.

Clinical Learning Strategy

  • Provide leadership and oversight for a unified clinical learning strategy that establishes a consistent Cityblock approach to patient care.
  • Partner with clinical leaders and subject matter experts to identify the knowledge, skills, and capabilities required across clinical roles.
  • Convene and facilitate internal and external subject matter experts to define clinical content and translate their expertise into clear, scalable learning priorities.
  • Establish learning priorities across providers, registered nurses, behavioral health, pharmacy, Community Health Partners, and Medical Assistants.
  • Partner with Learning and Development, Clinical Team Enablement, Provider Enablement, and Integrated Care to inform the sequencing and delivery of clinical learning.
  • Monitor progress on learning initiatives, identify dependencies and risks, and provide visibility to clinical leadership.
  • Support alignment of clinical learning priorities across continuing medical education, asynchronous learning, and market-based training.

Executive Leadership & Operating Cadence

  • Run the content and follow-through components of the Chief Health Officer's leadership operating cadence.
  • Own agendas, pre-reads, materials, and follow-up for key clinical leadership forums and quarterly goal reviews.
  • Maintain decision and commitment tracking and ensure follow-through on agreed actions.
  • Prepare materials and insights for executive leadership, board, and enterprise leadership forums.
  • Support the annual goal cascade from Chief Health Officer priorities through VP and frontline leadership levels.
  • Design and support clinical leadership offsites and strategic meetings.
  • Identify cross-functional issues that require executive attention and ensure appropriate escalation and resolution.

What You’ll Bring

  • 8+ years of progressive leadership experience in clinical operations, healthcare operations, or a related field, including significant experience operating at scale.
  • Demonstrated experience leading clinical performance management, operational improvement, and workforce or provider operations.
  • Experience working across multiple markets or states with varying regulatory, payer, and operational environments.
  • Strong understanding of clinical performance data and the ability to translate complex data into actionable insights.
  • Experience partnering directly with analytics and data teams to define metrics, develop reporting, and establish performance measurement.
  • Demonstrated ability to lead complex, cross-functional initiatives without direct authority over all stakeholders.
  • Strong executive presence and the ability to build credibility and influence with physician, clinical, operational, and executive leaders.
  • Experience convening subject matter experts and translating complex clinical information into clear strategies, priorities, and actions.
  • Experience working with Medicaid and/or dual-eligible populations and members with complex medical, behavioral, and social needs.
  • Strong strategic thinking, organizational skills, and ability to operate effectively in a fast-paced, evolving environment.
  • Clinical licensure is not required; demonstrated credibility with physician and advanced practice clinician leaders is important.

Preferred Qualifications

  • Experience in a value-based care, downside-risk, or full-risk healthcare organization.
  • Experience with total cost of care, risk adjustment, quality performance, medical spend, or related healthcare financial metrics.
  • Experience with clinical education, provider enablement, competency modeling, or continuing medical education.
  • Experience partnering with actuarial, health economics, finance, or value creation teams.
  • Experience working with product and technology teams, including clinical technology prioritization, requirements definition, or workflow improvement.
  • Experience with specialty network strategy or value-based specialty contracting.
  • Experience in behavioral health and/or clinical pharmacy operations.

Success in This Role

Success in this role will be demonstrated by the clinical organization having greater visibility into performance, stronger execution against cross-functional priorities, and a consistent operating cadence that enables leaders to identify issues early and act on them.

The Director will help ensure that clinical, financial, workforce, and operational insights are connected and translated into meaningful action while maintaining clear accountability with the leaders who own those outcomes.

About Us:

Cityblock Health is the first tech-driven provider for communities with complex needs—bringing better care to where it’s needed most, block by block. Founded in 2017 on the premise that “health is local” and based in Brooklyn, we are backed by Alphabet’s Sidewalk Labs along with some of the top healthcare investors in the country.

Our mission is to improve the health of underserved communities. Importantly, our solutions are designed specifically for Medicaid and lower-income Medicare beneficiaries, and we meet our members where they are, bringing care into the home and neighborhoods through our community-based care teams and Virtual Care offerings.

In close collaboration with community-based organizations, local providers, and leading health plans, we are reorganizing the health system to focus on what matters to our members. Equipped with world-class, custom care delivery technology, we deliver personalized primary care, behavioral health, and social services to deliver a radically better experience of care for every member and community we serve.

Over the next year, we’ll grow quickly to bring better care to many more members and their communities. To do this, we need people who, like us, believe that everyone should have good care for what matters to them, in their community.

Our work is grounded in a belief in the power of a diverse community. To close gaps in care and advance equity in the communities we serve, we have to start with making our own team diverse and inclusive. Our ways of working are characterized by creativity, collaboration, and mutual learning that comes from bringing together a community from diverse backgrounds and perspectives. We strive to ensure that every person on the Cityblock team, and every Cityblock member, feels supported and included as a part of our community.

Our Values:
Aim for Understanding
Be All In
Bring Your Whole Self
Lean Into Discomfort
Put Members First

About our Team:
We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. We will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.
