Clinical Care Coordinator

The CCC maintains an assigned caseload and provides medically necessary clinical care coordination consistent with the individual's diagnosis, assessed needs, treatment plan, level of care, and applicable District of Columbia Department of Behavioral Health (DBH) requirements.

The position is designed as a productive clinical role. The CCC is expected to provide and document billable services throughout the workweek while maintaining high standards for clinical quality, medical necessity, documentation, compliance, and continuity of care.

The CCC works closely with LES clinicians, counselors, nurses, Advanced Practice Nurses, Recovery Support Specialists, treatment teams, billing personnel, and external healthcare and community providers.
