About the Role
We are seeking a detail-oriented, compassionate, and clinically skilled Registered Nurse (RN) Clinical Intake Specialist to join our patient care team. In this pivotal role, you will serve as the initial clinical touchpoint for incoming patients, evaluating medical histories, assessing urgency, and ensuring seamless onboarding into our care pathways.
The ideal candidate possesses strong clinical assessment skills, excellent communication abilities, and a patient-centered mindset. Experience in oncology or complex care management is strongly preferred, as many of our cases involve detailed medical reviews and coordination for specialized treatment plans.
Key Responsibilities
- Clinical Assessment & Triage: Conduct thorough phone or virtual clinical evaluations of incoming patient referrals, medical records, and diagnostic reports to determine urgency and care needs.
- Onboarding & Care Coordination: Facilitate the intake process by coordinating with referring physicians, specialists, and multidisciplinary teams to ensure a smooth transition of care.
- Patient & Family Education: Serve as a empathetic advocate, guiding patients and their families through the initial intake process, explaining care protocols, and addressing initial questions or concerns.
- Documentation & Compliance: Accurately chart clinical notes, medical histories, and triage findings in the Electronic Health Record (EHR) system, adhering to HIPAA and organizational quality standards.
- Specialized Case Evaluation: Review complex clinical histories—particularly cancer care diagnoses, lab results, and treatment regimens—to route patients to the appropriate specialists efficiently.
Qualifications
- Licensure: Active, unrestricted Registered Nurse (RN) license in the state of practice (Compact license preferred).
- Education: Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Experience:
- Minimum of 2–3 years of clinical nursing experience (intake, case management, triage, or ambulatory care experience preferred).
- Oncology experience (inpatient, outpatient, or navigation) is strongly preferred.
- Technical Skills: Proficiency with Electronic Health Record (EHR) systems (e.g., Epic, Cerner) and general digital communication tools.
- Soft Skills: Exceptional active listening, verbal/written communication, time management, and empathetic patient engagement.
