About the Role

We’re hiring an RN Care Navigator to work directly with patients while bringing clinical experience and judgment to our growing Care Navigation team.

This is a hands-on, patient-facing role for an RN who enjoys building relationships, coordinating care, and helping patients navigate complex healthcare needs. You’ll manage your own patient caseload while also serving as an accessible clinical resource for Care Partners when questions or more complex patient situations arise.

You’ll also have the opportunity to help improve the workflows, protocols, and tools that support safe, consistent, high-quality patient care as Nurra grows.

What You’ll Do

  • Build trusting relationships with patients through consistent, compassionate outreach

  • Help patients navigate appointments, referrals, medications, care plans, and next steps

  • Coordinate across providers, specialists, pharmacies, and healthcare systems to keep patient needs moving forward

  • Provide high-touch support that makes patients feel seen, heard, and cared for

  • Manage a patient caseload with strong documentation, organization, and follow-through

  • Serve as a clinical resource for Care Partners when clinical questions or more complex patient situations arise

  • Support questions related to medications, medication reconciliation, changes in patient condition, and appropriate escalation or next steps

  • Help develop clear, practical guidance for common clinical workflows, including intake, care planning, medication reconciliation, and clinical escalation

  • Identify recurring clinical questions or challenges that could be addressed through better workflows, training, or guidance

  • Partner with Clinical Operations and clinical advisors to translate clinical guidance into practical, day-to-day processes

  • Adapt as our tools, workflows, and internal systems evolve

What We’re Looking For

  • Active RN license in good standingn (CA)

  • Experience providing direct patient care

  • Strong clinical judgment and the ability to recognize when concerns require escalation

  • Ability to communicate clinical information clearly and practically to both patients and team members

  • Excellent communication and interpersonal skills

  • Genuine empathy and a strong instinct for patient-centered care

  • Strong organizational skills and attention to detail

  • Comfort working across multiple digital tools and platforms

  • Ability to balance clinical thinking with warmth, efficiency, and follow-through

  • Resilience, flexibility, and adaptability in a fast-growing startup environment

Helpful Backgrounds

We’d especially love to hear from RNs with experience in:

  • Care management or care coordination

  • Chronic disease management

  • Complex care management

  • Telehealth or remote patient support

  • Patient education and medication reconciliation

Success in this role means patients feel genuinely supported, their care needs are followed through reliably, and clinical concerns are recognized and escalated appropriately.

The right person is both a strong RN and a strong relationship-builder, someone who can connect deeply with patients, navigate complex healthcare needs, and use their clinical experience to support thoughtful, high-quality care.
