Overview
The Community Health Associate (CHA) supports the BRAVE program by providing trauma-informed, client-centered behavioral health navigation and care coordination for students and families. The CHA receives and manages referrals, conducts intake and needs assessments, identifies appropriate behavioral health resources, facilitates connections to providers, addresses barriers to accessing services, and follows up with families, school personnel, and providers to support continuity of care. The position maintains accurate client records and works collaboratively with the BRAVE team and community partners.
BRAVE (Be Resilient and Voice Emotions) is a program under UF Health St. Johns Care Connect that was developed to address barriers to timely behavioral health services for students, including provider wait times and fragmented communication between schools, families, and behavioral health providers. Through a unified referral and intake process, BRAVE supports resource navigation, care coordination, referral follow-through, and communication among participating partners. The program supports the referral structure outlined in the Marjory Stoneman Douglas High School Public Safety Act.
