Job Summary: Position Type: Contract Anticipated Duration: 10 months (February 2025 - December 2025) Location: Midwest (50% onsite, 50% remote) The Healthy Planet Lead will manage the implementation of Epic's Healthy Planet module, which focuses on population health and care management. The role includes collaboration with clinical leaders to ensure the system meets operational and clinical goals. Responsibilities: - Lead the implementation of Epic’s Healthy Planet module. - Collaborate with population health teams to ensure the system meets clinical and operational needs. - Oversee system configuration, testing, and troubleshooting. - Provide post-go-live support and continuous improvement strategies. Requirements Requirements: - 5+ years of experience in healthcare IT, with a focus on population health. - Epic Healthy Planet certification required. - Proven experience in implementing and supporting population health solutions. Expected Travel: 50% onsite travel in the Midwest. Expected Schedule: Full-time, with additional hours during go-live phases.
