Convert Zoho Creator App to Web App -

Job Description 2016-04-01: We have an application built on Zoho Creator that has two HTML pages. The first page is a form with checkboxes. The second page is a results page from the checked criteria. The goal of this project is to convert the Zoho app to a web app that connects to a MySQL database.
