The Role To support our growth, we are looking to add a Director, Clinical Performance to our Clinical Performance team! We are looking for a motivated program leader who will play a key role in building and scaling Greater Good Health's new community care model — supporting primary care patients with wraparound care needs, including those who cannot easily reach the clinic. You will partner closely with Clinical Performance, Data Science, Finance, and market/clinic leadership to set program strategy, build the operating model, and drive performance across quality, transitions of care, and remote care coordination. Ideally, you have a strong background managing value-based care programs from design through deployment, including transitions of care, care management, or remote/telephonic care coordination operations. Responsibilities: Reports to SVP, Clinical Performance Owns the performance of a portfolio of Clinical Performance programs and central teammates to support that work Leads the build-out of GGH's new community care model Designs workflows for remote care coordination, care management, and transitions of care Ensures the model integrates cleanly with clinic-based care teams Partners with Analytics and Tech teams to build analytics and reporting that surface performance trends Pilots, measures, and scales new interventions based on results Sets and continuously refines program strategy in partnership with clinical, operations, data science, and finance leaders Builds the operating processes, staffing models, and management routines that keep programs accountable to outcomes Leads and develops the Clinical Performance team Delegates meaningfully and builds team capability, creating a strong bench for future growth Onboards and trains new teammates on programs, tools, and processes as the team grows Assesses and coordinates workloads across the team's program portfolio Reassigns priorities and staffing as programs scale or shift Works with Operations leadership to plan for coverage during time away or turnover Hosts team huddles and reviews at an appropriate cadence with the VP, Clinical Performance Runs and reviews performance reports to track program outcomes against targets Identifies trends and learning opportunities across the portfolio Ensures excellent care, high performance, and productivity across the portfolio, ensuring we exceed client and patient expectations Evaluates and implements technology and workflows to improve operational and team efficiencies Serves as subject matter expert on community care, transitions of care, and care coordination both internally and externally Collaborates with regional and central leadership by recommending strategies on growth, retention, professional development, and company-wide initiatives Builds business cases, ROI analysis, and financial analysis to support new program investments Follows all HR policies and regulations per state law (consult HR or Senior Ops as necessary) Performs additional duties as assigned Minimum qualifications Bachelor's Degree 7+ years of experience managing value-based care program development and deployment 3+ years of experience leading teams Experience managing remote Team Members in different time zones Strong business case, ROI, and financial analysis skills Excellent communication and stakeholder management skills, including with clinicians Highly motivated, adaptable, organized, self-starter, collaborative, and trustworthy Computer literate; proficient in Microsoft Office applications (Word, Excel, PowerPoint) Preferred qualifications Experience managing transitions of care, care management, or remote/telephonic care coordination operations Advanced degree (MPH, MHA, MBA, or clinical) Experience in a fast-growing, value-based care or healthcare services organization Perks and Benefits: Competitive Compensation Package: We offer a competitive compensation package to recognize your valuable contributions and ensure your financial security.
