Coordinate and ensure completion of initial and annual evaluations and assessments Facilitate and participate in developing the Individual Family Services Plan (IFSP) to support the child and assist the family in monitoring IFSP implementation and progress Provide ongoing advocacy support to ensure that appropriate EI services are being provided by communicating monthly with the family, using a variety of mechanisms, including face-to face visits Maintain accurate documentation in the form of case notes Facilitate the development of a transition plan to preschool, school, or if appropriate, to other services. Requirements Bachelor’s Degree in Human Services or a related field (Required) Birth to age 5 Case Management experience (Preferred) Minimum one-year experience working with IDD and/or disabled population (Preferred) Early Intervention Credentialed (Preferred) Early Intervention Credential (Completion Required within 3 months of hire date) Valid Driver’s License, Insurance, and reliable transportation (Required) Ability to meet Essential Job Functions (with or without reasonable accommodation) (Required) Bilingual encouraged to apply Please submit Resume and Cover Letter for consideration.
