As an ECM Lead Care Manager (LCM) Team Lead, you will play a key role in supporting frontline operations, strengthening team performance, and reinforcing high-quality, person-centered care delivery across the department. This position is ideal for an experienced ECM professional who thrives in both field-based member support and team development. Why This Role Matters The ECM LCM Team Lead serves as a bridge between frontline staff and leadership, helping ensure operational consistency, accountability, and compassionate member care. In addition to managing a reduced caseload, this role provides hands-on support to fellow LCMs, assists with onboarding and coaching, and helps maintain department workflows and quality standards. This role combines direct member impact with operational leadership and is ideal for someone passionate about mentorship, problem-solving, and improving outcomes for vulnerable populations. What You’ll Do Team Support & Operations Provide day-to-day support and guidance to fellow Lead Care Managers (LCMs) Serve as a resource for staff questions, workflows, and ECM best practices Assist in monitoring team productivity, documentation completion, and operational consistency Identify and escalate concerns, risks, workflow gaps, or trends to management as appropriate Support coverage needs, including member follow-up when assigned LCMs are unavailable (e.g., PTO coverage) Collaborate closely with leadership to maintain accountability, performance standards, and quality of care delivery Reinforce company expectations, workflows, and compliance standards across the team Training & Staff Development Support onboarding and ongoing development of new and existing LCMs Provide shadowing opportunities during in-person visits and fieldwork Demonstrate best practices for member engagement, documentation, outreach, and care coordination Provide real-time coaching, guidance, and operational support to staff Assist with reinforcing documentation standards, productivity expectations, and quality assurance initiatives Care Coordination & Member Support Provide direct member support and care coordination assistance as assigned based on operational and coverage needs Assist with member follow-up, engagement, and continuity of care during staffing gaps, PTO coverage, or high-volume periods Support Lead Care Managers with complex member coordination, barriers to engagement, and escalation needs Coordinate medical, behavioral health, and community-based services to support member stability and care continuity Assist with transitions of care, hospital discharges, referrals, and follow-up coordination as needed Build strong, supportive relationships with members through compassionate, person-centered engagement Outreach Support Provide support for community-based outreach initiatives as needed Participate in occasional in-person outreach activities across multiple counties Assist with identifying, engaging, and enrolling eligible members into ECM, Community Supports (CS), and Behavioral Health (BH) programs Support community partnership development and outreach efforts Assist during coverage gaps, staffing shortages, or high-need operational periods Documentation & Compliance Maintain timely, accurate, and compliant documentation in accordance with Medi-Cal, CalAIM, and ECM requirements Assist in reinforcing documentation quality standards across the team Support quality improvement efforts and internal auditing processes Ensure care coordination activities are properly documented within company systems Minimum Qualifications 3+ years of experience in Enhanced Care Management (ECM), case management, social services, or healthcare coordination Previous experience mentoring, training, or supporting staff strongly preferred Strong understanding of Medi-Cal, CalAIM, and ECM program requirements Excellent communication, leadership, organizational, and problem-solving skills Ability to effectively balance member care responsibilities with operational support duties Strong documentation and technology proficiency Experience working with vulnerable or high-acuity populations preferred Who Thrives in This Role This role is a strong fit for someone who: Naturally mentors and supports others Leads through collaboration, accountability, and consistency Thrives in fast-paced, community-based environments Is proactive, adaptable, and solution-oriented Balances compassion for members with strong operational awareness Is comfortable stepping into both field-based and administrative responsibilities Takes initiative and helps strengthen team culture and performance How Success Is Measured Consistent support and guidance provided to LCM team members Timely and compliant documentation completion Strong member engagement and care coordination outcomes Positive collaboration with staff, leadership, providers, and community partners Ability to assist with maintaining team productivity, accountability, and operational consistency Effective onboarding and development support for new staff
