Help families navigate their child's early development and make a lasting difference in their lives!
Are you passionate about supporting children and families, building meaningful relationships, and connecting people with resources that help them thrive? Akin is looking for a compassionate, resourceful, and dedicated Family Resources Coordinator (FRC) to join our Early Support for Infants and Toddlers (ESIT) team!
In this role, you'll be a trusted partner to families navigating their child's developmental needs. You'll help caregivers understand available services, advocate for their children, and access the resources and support they need during some of the most important years of their child's development.
If you're someone who enjoys connecting with people, solving problems, and making a meaningful difference in your community, we'd love to hear from you!
About Akin
At Akin, we believe every child and family deserves the opportunity to thrive. We partner with children, families, and communities to build strong relationships, support well-being, and create opportunities for lifelong success.
Our Early Support for Infants and Toddlers (ESIT) program helps families with children from birth to age three who are experiencing developmental delays or disabilities. Through family-centered services, early intervention, and community partnerships, we help children build essential skills while empowering caregivers to support their child's growth and development.
When you join Akin, you become part of a collaborative team committed to strengthening families, celebrating individual strengths, and creating lasting positive change.
About the Role
As a Family Resources Coordinator, you'll serve as a primary point of contact and advocate for families receiving ESIT services. You'll work closely with caregivers to understand their goals, identify their child's strengths and developmental needs, and coordinate services that support the entire family.
You'll guide families through the early intervention process, help develop and monitor Individualized Family Service Plans (IFSPs), connect families with community resources, and collaborate with service providers to ensure children receive coordinated, timely support.
This is an opportunity to build meaningful relationships with families, help them navigate important decisions, and celebrate developmental milestones along the way.
What You'll Do
Partner with Families and Support Their Goals
-
Build trusting, supportive relationships with parents and caregivers, recognizing their strengths, priorities, and hopes for their children.
-
Help families identify their child's developmental strengths and needs and develop individualized goals through the Individualized Family Service Plan (IFSP) process.
-
Guide families through the ESIT program, ensuring they understand available services, their rights, and important procedural safeguards.
-
Maintain regular communication with families, including monthly check-ins, to provide guidance, answer questions, and ensure services continue to meet their needs.
-
Empower families to advocate for their children and make informed decisions about their child's development and care.
Coordinate Early Intervention Services
-
Coordinate the development, implementation, scheduling, and review of Individualized Family Service Plans.
-
Collaborate with early intervention specialists, referring agencies, healthcare providers, and community partners to ensure services are aligned with each child's needs.
-
Facilitate ongoing communication among families, service providers, and partner agencies, including required six-month reviews and updates regarding significant case developments.
-
Support families in navigating complex service systems and accessing additional programs, including Medicaid-related resources when applicable.
-
Maintain accurate, timely, and confidential case records and documentation.
Connect Families with Community Resources
-
Help families identify and access community resources that support their goals and overall well-being.
-
Develop and maintain strong relationships with local social service agencies, healthcare providers, and other family-serving organizations.
-
Coordinate referrals and connect families with additional services when needed.
-
Organize and participate in family events, parent meetings, and opportunities that encourage connection and engagement.
Collaborate and Grow with Your Team
-
Work closely with a multidisciplinary team of early intervention professionals to provide coordinated, family-centered support.
-
Participate in case reviews, team meetings, program planning, and other collaborative activities.
-
Support continuous improvement efforts that strengthen services for children and families.
-
Participate in ongoing professional development, including training related to child development, family support, HIPAA, child abuse awareness, antiracism, and other relevant topics.
-
Reflect on your work, seek opportunities for growth, and contribute to a positive, supportive team environment.
-
Perform other related duties as assigned to support effective program operations.
