Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live. What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Full-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are in-office at our Irvine headquarters for administrative and documentation tasks.
