Position Overview
The Health Economist / Costing Analyst provides technical leadership in health economics, costing, and financial analysis to strengthen the efficiency, sustainability, and financing of EGPAF-supported HIV, TB, and maternal, neonatal and child health (MNCH) programs. The Health Economist works within the Technical Excellence (TE) department, in close collaboration with the Monitoring, Evaluation and Learning (MEL) team, the Digital Health and Data Analytics (DHDA) team, and country program teams, to develop costed investment cases, assess the cost-efficiency of service delivery models, and inform domestic resource mobilization and transition planning in EGPAF-supported countries in sub-Saharan Africa.
This position's work is executed in alignment with EGPAF's 2026–2028 Strategic Direction — advancing triple elimination, ending pediatric AIDS, preventing maternal morbidity and mortality, child survival, and health system sustainability. Within this broad scope, the Health Economist will primarily support countries funded by the U.S. Department of State's Bureau of Global Health Security and Diplomacy (GHSD), beginning with Lesotho and Tanzania, in addition to other responsibilities within the global Technical Excellence department. The Health Economist contributes economic evidence and analysis to help countries sustain progress toward 95-95-95 outcomes for pregnant women, children, and adolescents and reach the WHO elimination milestones (mother-to-child transmission rates under 5%, fewer than 50 infections per 100,000 live births, and HBV prevalence under 0.1% in children under five).
This position is central to EGPAF's adaptation to the evolving global health financing landscape, including the U.S. Department of State's America First Global Health Strategy (AFGHS) and its emphasis on bilateral, co-financed country partnerships, milestone-based or performance-based funding, and transition planning to country ownership of health programs. The Health Economist develops the costed evidence base — investment cases, financing-gap analyses, and cost-efficiency assessments — to inform sustainability and effective HIV programs, and works closely with country program teams, other Technical Excellence staff, donors, and national Ministries of Health and other external partners on these efforts.
Key Responsibilities
Investment Case and Costing Analysis
- Lead the development of triple elimination investment cases in Lesotho, Tanzania, and other GHSD-supported countries, quantifying program costs, financing gaps, and projected health and economic returns to inform domestic resource allocation.
- Conduct costed analyses of prevention and case-management approaches, including PrEP (including long-acting options such as lenacapavir) and Mentor Mother peer-support and case-management models, to inform PMTCT retention strategies.
- Contribute costing and financial analysis to gap analyses on eMTCT and pediatric HIV, quantifying subnational financing and capacity needs to target tailored approaches.
- Provide costing inputs to national and subnational triple elimination validation dossiers and evidence packages.
Cost-Efficiency and Service Delivery Analysis
- Assess the cost efficiency of site-specific disease-integration models, such as integrating multiple disease areas into MCH platforms, integrating ART services into outpatient departments, and converting HIV clinics into chronic care clinics.
- Analyze cost drivers and efficiency gains associated with service delivery innovations, including hub-and-spoke PrEP models, technical hub models, and RAS (Regional Acceleration Strategy) implementation approaches.
- Provide technical costing input on commodity forecasting and quantification for integrated ANC test panels and other MCH and HIV commodities.
- Support unit-cost and cost-efficiency analysis of surveillance and Emergency Operations Center (EOC) investments.
Data, Tools, and Systems for Costing
- Develop and maintain standardized costing tools, templates, and methodologies for use across EGPAF-supported country programs.
- Support integration of costing and financial data into program dashboards and data-use platforms in collaboration with the MEL and DHDA teams.
- Ensure costing methodologies align with WHO, UNAIDS, and other recognized global standards for HIV, TB, and MNCH program costing.
Donor Engagement and Proposal Development
- Serve as technical writer and reviewer for costing, investment case, and sustainability-financing sections of proposals, concept notes, sole-source applications, and donor reports, including for GHSD-funded bilateral and co-financed mechanisms.
- Represent EGPAF's health economics and costing expertise in donor meetings and technical working groups related to America First Global Health Strategy implementation and domestic financing.
Capacity Building and Collaboration
- Build the capacity of country program staff and Ministry of Health counterparts in costing methodologies, investment case development, and financial planning.
- Facilitate and/or contribute to cross-country learning exchange on costing and health financing approaches.
- Collaborate closely with country teams to ensure costing and economic evidence are integrated into program design and technical assistance.
Key Deliverables
Under GHSD-funded and related EGPAF programming, the Health Economist / Costing Analyst is expected to produce the listed deliverables, in addition to achievement of specific annual targets and milestones defined in the award's PMEP and annual workplan.
- Investment cases — completed and approved triple elimination investment cases in GHSD-supported countries, quantifying program costs, financing gaps, and projected returns.
- Costing and cost-efficiency analyses — assessments of integrated service delivery, PrEP, technical hub, and RAS models to inform program design and resource allocation.
- Costing tools and guidance — standardized costing templates, tools, and methodological guidance for use across country programs.
- Proposal and donor deliverables — costing and investment case sections of proposals, concept notes, sole-source applications, and donor reports.
- Program management inputs — costing and financial inputs to annual work plans, M&E frameworks, and PMEPs required under donor agreements.
To be successful, you will have:
Required Qualifications
- Advanced degree (MA, MSc, or PhD) in Health Economics, Economics, Public Health, or a related field.
- Minimum of 7–10 years of progressive experience conducting costing, health economics, or investment case analysis for HIV, TB, or MNCH programs, preferably in sub-Saharan Africa.
- Demonstrated technical expertise in costing methodologies, such as activity-based costing, unit-cost analysis, and cost-effectiveness analysis, for health programs.
- Experience developing investment cases or financing strategies for donor-funded global health programs.
- Experience working with U.S. government (GHSD, CDC)-funded programs and navigating evolving global health financing mechanisms, including bilateral, co-financed donor agreements and sole-source award mechanisms.
- Experience supporting Ministry of Health transition/sustainability planning and domestic resource mobilization (e.g., under GHSD's America First Global Health Strategy).
- Strong technical writing skills, including experience contributing to proposals, technical briefs, and costing or investment case reports.
- Strong quantitative and data analysis skills, including experience with costing tools or models and the use of artificial intelligence (AI) in data analysis.
- Excellent interpersonal and cross-cultural communication skills, with the ability to work effectively across global, multi-country teams.
- Willingness and ability to travel internationally up to 30% of the time.
- Fluency in English required.
Preferred Qualifications
- Familiarity with digital health platforms (DHIS2, EMR systems) and AI-enabled decision-support tools, and their application to HIV, TB, and MNCH programming.
- Experience with WHO/UNAIDS costing and planning tools, such as the OneHealth Tool, Investment Case toolkits, or Spectrum/Goals model.
- Experience facilitating technical communities of practice or cross-country learning networks.
- Experience working in HIV, TB, or MNCH programs is an advantage.
- French or Portuguese proficiency an advantage.
