Qualifications: - Associates Degree - At least one year related work experience Duties and Responsibilities: Review records for completeness, accuracy and regulatory compliance. Extract relevant data from information provided by others, and input it into spreadsheets or standard formats. Follow a range of mandatory procedures to ensure that work is carried out to the required quality standards and/or to ensure adherence to external regulatory codes and internal codes of conduct. Enter data into standard company systems, and review and verify it for accuracy. Organize and prepare complex documents using a variety of applications for technology devices such as standard office software. Also responsible for gathering documents for release to external entities. Provide operational support by performing a range of route activities using existing systems and protocols. Manage basic aspects of the data management system with guidance from senior colleagues. This includes being responsible for developing or operating basic elements of the document management systems to deliver prescribed outcomes. Maintain an understanding of relevant technology, external regulation, and industry best practices. TASKS Complies with all rules, regulations, laws, and standards associated with the Health Insurance Portability and Accountability Act (HIPAA). Handles all requests for information from the Easterseals MORC, Inc. Health Record meeting the established turnaround times. Verifies authorizations are in place, signed by the appropriate individual/legal representative and documents are complete before they are released. Follow rules of minimally necessary when responding to a request. Retrieve electronic medical records for internal and external requests as appropriate (e.g., release of information, audits, review). Provides necessary medical information to internal sources in compliance with all laws, regulations, standards, policies, and protocols. Validates that all documents are properly filed within the electronic health record system. Upload documents to the appropriate document type/form within the electronic health record. Handles and processes all Release of Information requests in compliance with HIPAA regulations as well as all laws, regulations, standards, policies and protocols. Serves as an information resource by responding to requests for general and health record information received via phone, email, fax or in person. Facilitates data integrity of any and all health record documents uploaded to the E.H.R. Refuse to fraudulently assign diagnostic codes that do not conform to approved coding principles/guidelines. Performs quality checks on scanned health record information. Performs validation/verification for scanned images. Process and compile data on opportunities for improving clinical documentation submitted to HIM. All customer related services and reports are delivered within specific timeframes as established by standards of practice.
