POSITION SUMMARY

The Home Health Admissions Registered Nurse completes comprehensive admission assessments and OASIS documentation for patients entering home health services. The Admissions RN evaluates clinical needs, confirms eligibility and service requirements, initiates the individualized plan of care, coordinates with the physician and interdisciplinary team, and provides skilled nursing care and education in the patient's residence. This position promotes each patient's maximum potential throughout the home health episode and supports safe, timely transitions of care.

ADMISSIONS AND OASIS RESPONSIBILITIES

  • Complete timely and accurate start-of-care and other assigned OASIS assessments in accordance with physician orders, the plan of care, agency policy, and applicable requirements.
  • Perform comprehensive admission assessments, including medical history, current diagnoses, medications, functional status, psychosocial needs, home environment, safety risks, and caregiver availability.
  • Verify that the patient's clinical needs and ordered services can be safely and appropriately addressed in the home health setting and promptly report barriers or concerns.
  • Initiate the individualized plan of care in collaboration with the physician, patient, caregiver, and interdisciplinary team.
  • Complete medication reconciliation, identify discrepancies or potential concerns, and communicate needed clarification to the physician and clinical leadership.
  • Obtain and document required consents, patient rights acknowledgments, emergency planning information, and admission education within the RN's assigned responsibilities.

SKILLED NURSING CARE

  • Provide skilled nursing care to patients of all ages in their places of residence within the RN scope of practice and demonstrated competency.
  • Perform ordered treatments and procedures, which may include wound care, wound-vac therapy, IV therapy, venipuncture, medication management, disease-process education, and other authorized skilled services.
  • Assess the patient's condition, response to treatment, pain, risks, and progress and promptly report significant changes or abnormal findings.
  • Provide patient and caregiver education regarding diagnoses, medications, treatments, infection prevention, safety, emergency planning, and the plan of care.
  • Use equipment and supplies safely, effectively, and in accordance with infection-control and agency requirements.

CARE COORDINATION AND COMMUNICATION

  • Assume responsibility for coordinating admission-related care with the physician, interdisciplinary team, patient, caregiver, referral source, and agency departments.
  • Communicate admission findings, identified risks, requested orders, and service needs to the Director of Nursing, Clinical Manager, and assigned care team.
  • Coordinate ordered disciplines, services, supplies, equipment, laboratory needs, and follow-up care necessary to implement the plan of care.
  • Collaborate with the physician, clinical team, patient, and caregiver regarding goals, progress, discharge planning, and transition-of-care needs.
  • Participate in required daily, weekly, and monthly meetings, case conferences, and care-coordination activities.

DOCUMENTATION AND COMPLIANCE

  • Complete OASIS, admission, clinical, and care-coordination documentation accurately, completely, and within agency-required timeframes.
  • Maintain proficiency with the agency's electronic medical record systems, including WellSky/Kinnser and other designated platforms.
  • Document only care, assessments, communication, and services actually completed; never falsify, pre-chart, backdate, duplicate, or improperly alter records.
  • Protect patient confidentiality and electronic access credentials in accordance with HIPAA, agency policy, and applicable requirements.
  • Participate in quality assurance, performance improvement, utilization review, training, competency, and audit activities as assigned.

PROFESSIONAL CONDUCT AND ACCOUNTABILITY

  • Maintain dependable attendance, timely communication, professional boundaries, and respectful interactions with patients, caregivers, physicians, referral sources, and agency staff.
  • Exercise sound clinical judgment, attention to detail, organization, and independent decision-making within the RN scope of practice.
  • Promptly report incidents, safety concerns, suspected abuse or neglect, medication issues, missed services, documentation barriers, and changes in patient condition.
  • Maintain all required licenses, certifications, health screenings, competencies, and professional qualifications throughout employment.
  • Perform other job-related duties assigned by the Director of Nursing, Clinical Manager, Administrator, or agency leadership.

REQUIRED QUALIFICATIONS

  • Current, unrestricted license as a Registered Nurse in the State of Texas or valid authority to practice in Texas under the Nurse Licensure Compact.
  • At least two years of Registered Nurse experience in a home health setting.
  • Demonstrated experience completing OASIS admissions and start-of-care assessments.
  • Graduate of an accredited diploma, associate degree, or baccalaureate nursing program.
  • Current CPR certification and current health-screening documentation required by the agency, including a negative TB screening and Hepatitis B vaccination, immunity, declination, or other required documentation.
  • Current and valid Texas driver's license and access to a dependable automobile maintained in good working order.
  • Current automobile insurance that meets Texas and agency requirements.
  • Strong nursing assessment, clinical documentation, communication, interpersonal, time-management, and organizational skills.
  • Dependable, compassionate, attentive to detail, and committed to high-quality home health care.
  • Satisfactory work history, professional references, background screening, and other agency compliance requirements.

PREFERRED QUALIFICATIONS

  • Experience with WellSky/Kinnser and other home health electronic medical record systems.
  • Bilingual communication skills in English and Spanish.
  • Experience with wound care, wound-vac therapy, IV therapy, venipuncture, and complex patient education.

PHYSICAL AND TRAVEL REQUIREMENTS

The Admissions RN must be ambulatory and able to travel to patient residences throughout the agency's assigned service areas and to the Dallas office as required. The position requires the ability to climb stairs; stand, walk, bend, reach, and kneel; push and pull; carry nursing supplies and equipment; and safely assist or facilitate patient movement within the RN's training, demonstrated competency, plan of care, and agency policy. PERFORMANCE STANDARDS

  • Complete assigned admissions and OASIS assessments accurately, thoroughly, and within required timeframes.
  • Develop and communicate a complete, patient-specific plan of care based on assessment findings, physician orders, and identified needs.
  • Provide skilled nursing care safely, accurately, respectfully, and within the Registered Nurse scope of practice.
  • Promptly communicate changes in condition, admission barriers, requested orders, safety concerns, and coordination needs.
  • Maintain accurate clinical documentation, dependable communication, patient confidentiality, and professional conduct at all times.
