Billet Health is looking for a compassionate, skilled, and dedicated Physical Therapist (PT) to join our growing Home Health team serving patients throughout the Phoenix Valley.

This per diem opportunity offers flexibility, autonomy, and the opportunity to provide one-on-one skilled care directly in patients’ homes—helping them regain mobility, improve function, and maintain independence.

Your Role

As a Home Health Physical Therapist, you will evaluate and treat patients in their homes while developing individualized plans of care based on each patient’s needs and physician orders.

Responsibilities

• Perform comprehensive patient evaluations and assessments
• Develop individualized treatment plans based on physician orders
• Provide skilled physical therapy services in patients’ homes
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on exercises, mobility, fall prevention, and safety
• Collaborate with physicians and interdisciplinary care team members
• Maintain accurate, complete, and timely clinical documentation
• Promote patient independence and safe mobility in the home environment

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Arizona Physical Therapist license
• Valid driver’s license and reliable transportation
• Current auto insurance
• CPR or BLS certification
• Current TB documentation
  • Skin test within one year, or
  • Chest X-ray within five years
• Home Health experience preferred
