This role is intake-focused and does not own provider matching, clinical assessment, case management assignment, billing decisions, or broad cross-department coordination.
POSITION SUMMARY & PURPOSE
The Intake Specialist serves as PBHC's front-end intake role for referral receipt, intake documentation tracking, initial client/referral-source outreach, service-type clarification, intake readiness updates, and structured handoff preparation under the operational access oversight of the CCO.
Intake support: the process of receiving, documenting, tracking, and following up on new client or referral information so the intake can move to the next approved step.
Referral follow-through: the communication and tracking needed to make sure referrals are not lost, delayed, duplicated, or left incomplete.

This role oversees:
- referral receipt and intake tracking
- intake packet and missing-document follow-up
- initial referral-source and client outreach related to intake completion
- stale referral and incomplete referral cleanup

Success in this role is measured by accurate intake tracking, timely outreach, clear documentation, reliable follow-through, appropriate escalation, and staying within the intake-only role lane during routine operations, growth, disruption, staffing transition, or organizational change.
