About the Role

Astrana is growing through acquisition and partnership. The Integration Lead owns that problem. You will run post-close integration for acquired entities and lead implementation for new partnerships and market launches, from signing through steady state. You will stand up the integration management office for each transaction, define the workstreams, hold the functional owners accountable to a plan, surface risks to the executive team early, and personally close the gaps nobody else owns.

This is an individual contributor role with no direct reports. Your leverage comes from influence over functional owners, not from headcount. This is not a coordination role either. You will build the Day 1 and Day 100 plans, reconcile the provider roster, pressure-test the synergy model, draft the physician communication, sit in the health plan delegation audit, and present the status readout to the CEO and CFO. The right candidate moves comfortably between those altitudes in the same week and is curious enough to go learn whatever the next workstream requires.

You will report to the Vice President of Strategy & Partnerships and work daily with Finance, Provider Contracting, Network Management, Claims, Health Plan Operations, Clinical and Quality, Risk Adjustment, Technology, Legal, Compliance, and People, as well as the leadership teams of the organizations we acquire and partner with.

Our Values
  • Put Patients First
  • Empower Entrepreneurial Provider and Care Teams
  • Operate with Integrity & Excellence
  • Be Innovative
  • Work As One Team

What You'll Do

Lead end-to-end integration of acquired entities
  • Build and run the integration management office (IMO) for each transaction: integration charter, governance cadence, workstream structure, decision rights, and escalation paths
  • Develop Day 1, Day 100, and 12-month integration plans with clear milestones, owners, dependencies, and critical path; manage transition services agreements (TSAs) through exit
  • Own the integration budget and synergy tracker; partner with FP&A to validate synergy assumptions, track realization against targets, and flag variances to leadership
  • Run weekly workstream reviews and biweekly executive steering committee meetings; prepare board-level and executive readouts on status, risks, and decisions required
  • Participate in pre-close diligence so that the integration thesis, risks, and resource needs are understood before signing, not discovered after
Drive the functional workstreams that make a managed care integration work
  • Provider network and contracting: migration of PCP and specialist agreements, division of financial responsibility (DOFR) alignment, capitation and fee schedule transitions, provider roster reconciliation and credentialing continuity
  • Health plan and delegation: transition of delegated functions (UM, credentialing, claims, care management), delegation agreement amendments, health plan notifications, and readiness for delegation oversight audits; regulatory filings with DMHC, CMS, and other state regulators as required
  • Finance and claims: claims run-out and system cutover, IBNR and capitation payment handoffs, bank and vendor transitions, entity and intercompany structure changes with Accounting and Legal
  • Clinical, quality, and risk adjustment: continuity of HEDIS and Stars reporting, care gap programs, supplemental data submission, risk adjustment documentation workflows, and care management caseloads through the transition
  • Technology and data: provider, member, and eligibility data migration; EHR, claims platform, and analytics integration; onboarding to Astrana's provider-facing tools
  • People and change management: provider and staff communications, town halls, key-person retention, role mapping, and culture integration with the People team
Lead implementation for new partnerships and market launches
  • Translate signed partnership and management services agreements into implementation plans: service scope, staffing, systems, reporting, and performance targets
  • Onboard new IPAs and provider groups onto the Astrana platform, including MSO service standup, data connectivity, quality and risk adjustment program launch, and provider engagement
  • Support the build of repeatable market-entry and affiliation playbooks so that each new partnership launches faster and cleaner than the last
Build the capability, not just the project
  • Develop and maintain Astrana's integration and implementation playbook: standard workplans, checklists, templates, KPIs, and lessons-learned reviews
  • Identify where integration repeatedly stalls and fix the underlying process, tooling, or ownership gap
Operate at every altitude
  • Present to and take direction from C-level executives; translate executive intent into workstream-level action
  • Produce your own work product: financial models, data reconciliations, process maps, communication drafts, and status materials
  • Other duties as assigned

Qualifications

  • Bachelor's degree in business, healthcare administration, public health, finance, or a related field, or equivalent experience
  • At least 6 years of progressive experience in healthcare operations, strategy, management consulting, corporate development, or a transformation / PMO function or similar functions
  • Direct experience leading at least one end-to-end M&A integration, large-scale implementation, or enterprise transformation with multiple concurrent workstreams and cross-functional owners
  • Demonstrated ability to work with executive leadership: structuring decisions, delivering difficult status updates, and gaining alignment across functions and people you do not manage
  • Strong analytical and financial fluency: comfortable building and auditing models in Excel, reading a P&L, and interrogating a synergy case
  • Excellent written and verbal communication; able to produce executive-ready materials without heavy editing
  • Proven ability to operate in ambiguity, manage competing priorities, and deliver on deadlines in a fast-paced environment
  • Ability to travel up to 25%, including to acquired and partner organizations in and outside California
Preferred
  • Experience in managed care, delegated risk, IPA / MSO, or value-based care environments; familiarity with capitation, DOFRs, health plan delegation, HEDIS / Stars, and Medicare Advantage risk adjustment
  • Experience with corporate practice of medicine structures, Knox-Keene or other state health plan licensure, and physician organization governance
  • Background in management consulting, corporate development, or an internal strategy / chief of staff function
  • Experience with claims and care management platforms (e.g., EZCAP) and provider data systems
  • MBA, MHA, MPH, or similar advanced degree
  • PMP, Lean / Six Sigma, or Prosci change management certification

Environmental Job Requirements and Working Conditions

  • Periodic travel to acquired and partner organizations, including multi-day site visits during integration milestones.
  • The total compensation target pay range for this role is between $140,000 - $160,000. This salary expectation represents our national target range for this role
