Position Overview: Deliver goal-directed, time-limited psychotherapy and guide collaborative service matching for clients entering specialized recovery services. Utilize evidence-based interventions tailored to co-occurring disorders, such as CBT, DBT-informed practices, Motivational Interviewing, ACT, CPT, and trauma-informed care. Coordinate closely with Medication-Assisted Treatment (MAT) teams, primary care, psychiatry, and case managers to provide holistic, integrated care. Leverage robust clinical capabilities in substance use and mental health treatment within a fast-paced Federally Qualified Health Center (FQHC). Eligible for a hybrid work model following successful onboarding and training period. Psychotherapy & Clinical Interventions Offer family, couples, and individual psychotherapy addressing concurrent mental health and substance use conditions. Facilitate episode-based, goal-oriented therapy that incorporates structured step-downs and collaborative discharge workflows. Implement moderation management frameworks, respecting patients' readiness levels and supporting incremental milestones. Perform comprehensive initial intakes and behavioral health assessments, leveraging standardized diagnostics (e.g., PHQ-9, GAD-7, AUDIT, DAST) and risk screenings for suicide, withdrawal, overdose, and psychosis. Lead evidence-based process and psychoeducational groups. Participate in the continuous design and updating of group curricula. Substance Use & Co-Occurring Disorder Care Execute target assessments for substance misuse and co-occurring issues, deploying risk evaluations and standardized screening measures. Rely on ASAM criteria to guide placement decisions and execute referrals to specialized care tiers when needed. Offer non-judgmental re-engagement, proactive safety planning, and treatment recalibration for patients experiencing a return to use. Distribute overdose prevention tools and education, including naloxone instruction and fentanyl testing info, across all stages of client readiness. Collaborate with psychiatric clinicians regarding Medication-Assisted Treatment (MAT). Programmatic & Professional Responsibilities Partner with multidisciplinary staff members across primary care, psychiatry, MAT, case management, peer support, Enhanced Care Management (ECM), and external care networks. Execute smooth warm handoffs across department lines. Engage in weekly clinical consultations and multidisciplinary team huddles to review cases. Maintain documentation practices within Epic, completing progress notes, Collaborative Care Plans, and discharge summaries timely. Execute mandatory consents and administrative program enrollment forms. Help satisfy grant requirements and quality performance metrics (e.g., HRSA, SAMHSA, Medi-Cal, HCAI). Assist in optimizing clinic workflows and supporting program enhancement projects. Deliver immediate crisis intervention support as necessary. Advocate for patients to dismantle institutional barriers and promote interdisciplinary, collaborative health management. Actively join month department meetings, bi-annual all-provider assemblies and internal in-service clinical training sessions. Uphold and advocate for the organization's administrative guidelines and policies. Pursue continuing professional education to maintain licensure, clinical competency, and relevant knowledge. Participate in regular performance reviews throughout employment. Bilingual candidates (English plus Russian, Ukrainian, Farsi, or Dari) are highly preferred, and while entry-level applicants are welcome, those with prior experience are strongly encouraged to apply. Requirements Active Clinical Social Worker license issued by the California Board of Behavioral Sciences, alongside a Master’s degree in Social Work from a program accredited by the Council on Social Work Education. Maintain a valid CPR (BLS) certification. Proven clinical experience in short-term, solution-focused, and evidence-based counseling, encompassing individual and group psychotherapy and the formulation of comprehensive psychosocial assessments and treatment plans. Certified or specially trained in a range of evidence-based therapeutic modalities, including Dialectical Behavior Therapy (DBT), Cognitive Behavioral Therapy (CBT), Cognitive Processing Therapy (CPT), Acceptance and Commitment Therapy (ACT), Parent-Child Interaction Therapy (PCIT), Eye Movement Desensitization and Reprocessing (EMDR), and/or motivational interviewing. Demonstrated expertise in crisis intervention techniques. Credentials or specialized education in substance use disorders, such as a Certified Alcohol and Drug Counselor (CADC) or Substance Use Disorder Certified Counselor (SUDCC) designation is highly desired. Extensive knowledge of psychosocial, psychiatric, ethical, legal, and confidentiality regulations; evaluation and treatment of chemical dependency and substance misuse; DSM-V diagnostic criteria; and mental health dynamics in children and adolescents. Familiarity with current mental health insurance regulations, benefits, and limitations. Profound comprehension of human behavior, case management strategies, therapeutic communication, and the direct impact of spiritual and cultural values on clinical practices. Understanding of integrated behavioral health practices within community clinical environments, with prior practical experience preferred. Capability to provide clinical supervision to Associate Clinical Social Workers (ASWs), if properly qualified. Additional Requirements: Must have a current and valid California driver’s license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours. The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
