DEPARTMENT : Cameron Psychiatry JOB TITLE : Licensed Clinical Social Worker or Licensed Social Worker SHIFT: 80 Hours Bi-Weekly, Monday - Friday. This is an onsite/hybrid job. BENEFITS : This is a full time, benefit eligible position. Summary/Objective Provides Medical direction and treatment of patients within the practitioner’s scope of practice and in collaboration with the supervising physician. Essential Functions Exemplifies good customer service. Collaborates with Multidisciplinary team in office and hospital to assure continuity of patient care. Conducts a comprehensive assessment of patients, interprets medical histories, identifies problems, and formulates a plan of treatment per standard of care. Documents patient care services in the patient’s EMR and discharge instructions in a timely manner. Participates in process improvement. Attends and actively participates in hospital and clinic-based meetings. Carries out all duties in a manner consistent with Cameron’s guiding principles and values and according to Cameron’s policies and procedures. Adheres to the Medical Staff Bylaws, Rules & Regulations, as appropriate Qualifications Experience Two years’ experience in designated clinic preferred Skills/Competencies Strong communication and critical thinking skills Ability to multitask and problem solve Education Required Graduate of an accredited social worker program Master’s degree required Licenses Requirements Current License in the State of Indiana Must be able to meet Cameron Hospital’s credentialing requirements Complete a CAQH and update annually as required Certifications Required or Preferred Clinical certificate required
